Neither unsolicited proposals nor any other kind of offers will be considered in response to this Sources Sought. Responses to this notice are not offers and will not be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this Sources Sought. All information received in response to this RFI that is marked Proprietary will be handled accordingly. Responses to the Sources Sought will not be returned. At this time, questions concerning the composition and requirements for a future Request for Quote will not be entertained. Subject: The Department of Veterans Affairs is conducting market research in order to provide a service agreement for the Louis A. Johnson VAMC (VAMC), 1 Medical Center Drive, Clarksburg WV 26301. The government anticipates awarding a Firm Fixed Price multiple year award contract that will provide a service agreement for one Hobart dishwasher, model FT900S. see below attachment. OBJECTIVE: See attachment. RESPONSE INSTRUCTIONS: Name and Address of Company. Any small business designation as certified by the Small Business Administration. A through description of experience in managing support requirements of this nature. 5.0 CONTACT INFORMATION: Any questions related to this Sources Sought shall be directed to mark Smith at Mark.Smith25@va.gov All information regarding Capabilities Statements or any other proprietary information relative to this Sources Sought shall be submitted via email to Mark.Smith25@va.gov no later than 3:00 pm EST on 4/08/2019 ATTACHMENT 1 Clarksburg STATEMENT OF WORK General Intention: Planned Service Agreement Contract for one Hobart dishwasher, model FT900S, S/N 271195325, located in the Nutrition and Food Service spaces. Includes Monday thru Friday, 8am 5pm coverage for parts, travel and labor, when repairs are required. Also includes two onsite visits per calendar year to perform two annual tune-ups, which includes preventive maintenance parts replacement. Place of Performance: Louis A. Johnson VAMC (VAMC), 1 Medical Center Drive, Clarksburg WV 26301 Performance Requirements: The services to be provided by the contractor under this agreement shall consist of furnishing all equipment, tools, labor, and transportation to maintain the Hobart dishwasher in fully operational condition. Each semiannual proactive maintenance visit includes: Verify proper operation of the equipment. Semiannually, replace the following parts: Wash arm O rings, float ball assembly, drain O rings, vacuum breaker kit and Hobart OEM curtains. Check the following: Control box, electrical components, fill & rinse system, conveyor & drive system, and heat system. Tune up the machine to ensure it is operating at peak performance. Unscheduled, call-in site visits shall include the following: Unlimited service calls and repairs. All labor, parts and travel costs shall be borne by the Contractor. Contractor hours of work: Contractor shall perform all work activities during normal facility business hours from 8am -5pm Monday Friday. Contractor will not be permitted to perform any work on federal holidays. Contractor will give the assigned COR two days advance notice to coordinate the planned, semi-annual site visits. Period of performance: This contract shall be for a base period of 12 months. The government at its sole discretion may elect to extend this contract for four additional 12month option periods. Contractor shall not proceed with work beyond the end of a 12month contract period, without prior written consent from the government.
