Page 1 of 2 THIS REQUEST FOR INFORMATION (RFI) SOURCES SOUGHT IS ISSUED SOLELY FOR MARKET RESEARCH AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. 1. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research only to make appropriate acquisition decisions and to gain knowledge of Small Business Capability, including Service Disabled Veteran Owned Small Businesses and Veteran Owned Small Businesses (SDVOSB/VOSB) interested and capable of performing the work. 2. The NAICS for this requirement is 515210 Cable and Other Subscription Programming Contractor shall provide customized Veteran/Patient Health Education TV Channel Monthly Subscription Services for the VA San Diego Healthcare System (VASDHS) located at 3350 La Jolla Village Dr. San Diego, CA 92161 and the Oceanside Outpatient Clinic located at 1300 Rancho del Oro Drive, Oceanside, CA 92056. The contractor shall be required to provide all necessary hardware and installation to receive the programming services of The Veterans Health Education Channel. The Contractor shall be required to provide all necessary equipment needed to project The Veterans Health Education Channel on the televisions at both locations. The contractor shall be required to provide a Veterans television channel programming service and signal in order to be accessed and viewed on all television drops in all the inpatient rooms, waiting areas and classrooms at both locations. The Contractor shall be responsible for putting together a detailed work plan and briefing. A Veteran specific custom health education channel via the VASDHS, Television system with 24 hours/7 days programming and VA Oceanside television systems with clinic channel loop 6 hours. All programs shall be broadcast in English with closed captioned in English. Closed caption shall also be available in Spanish when requested. Printed program schedules, posters, table tents, and promotional materials. Flyers, posters, schedules, table tents, etc. to support the Veterans Health Education Channel. The program schedule guide shall be printed in color and shall be updated quarterly. At minimum, program schedules supplied quantity of 5 times per bed count shall be provided on a quarterly basis with the ability to order more if needed. Minimum quantities of promotional materials if needed per quarter for VASDHS: 5 posters, 2500 flyers, 1000 food tray liners, and 500 table tents. Minimum quantities of promotional materials if needed per quarter for VA Oceanside: 10 posters, 400 flyers, and 100 table tents. All promotional materials shall also be provided in an electronic format to display on electronic boards. Programming health education topics that support VA 9 Healthy Living Message: Be Involved in Your Health Care, Be Tobacco Free, Eat Wisely, Be Physically Active, Strive for a Healthy Weight, Limit Alcohol, Get Recommended Screening Tests and Immunizations, Manage Stress and Be Safe Health education topics that support VA Performance Measures: Immunizations-including flu vaccine and pneumonia vaccine, Medication Reconciliation, Hypertension blood pressure checks, Hyperlipidemia, Women s Health, Diabetes, Colorectal Cancer Screening, Prostate Cancer Screening, Alcohol Use, Depression, Post Traumatic Stress Disorder (PTSD), Fall Prevention, Tobacco Cessation, Healthy Weight, Whole Health, and Pain Management Health Education topics to meet National Safety Goals: Medication Safety, Prevent Infection, Suicide Prevention, Prevent Mistakes in Surgery The TV programming signal shall be continuous, non-interrupted and with good viewing quality. The contractor shall take necessary action including testing, and regular maintenance to prevent and eliminate any signal interference or failure. The contractor shall provider all necessary repairs or replacements to the equipment within 3 working days of receiving notice or service call from VASDHS. The contractor shall provide a Train the Trainer for the VASDHS and Oceanside Clinic if needed. Provide access to programming vides and transcripts via website. Access to annual viewership study of the health education programing. The contractor at its sole expense, shall provide all repairs unless failure was due to a reason for which the VASDHS and Oceanside Clinic required to provide indemnification. The contractor shall provide technical support at no additional costs to the Government. The contractor shall be required to design, install, operate and maintain the TV programming system, wiring and connections in a safe, orderly and attractive manner. The contractor shall also design and install the system wiring and connections in a reliable manner and in conformance with local, state and federal regulations. The contractor shall ensure the system is designed, installed and operated in a manner that meets or exceeds all the applicable national electrical codes and the technical standards as required by the Federal Communications Commission (FCC). When directed by the contracting officer or designated Government representative, the contractor at its expense shall remove, replace, or modify any equipment including receivers, modulators, drop connections, and wiring that shall be determined to be unsafe, unsightly or otherwise unsuitable. 3. Interested and capable Contractors should respond to this notice not later than Tuesday, April 9, 2019 at 4:00 PM PST by providing the following via email only to Imelda.martinez@va.gov (a) company name (b) address (c) point of contact (d) phone, fax, and email of primary point of contact (e) DUNS number (f) Type of small business, if applicable, (e.g. Services Disabled Veteran Owned small Business(SDVOSB), Veteran-owned small business (VOSB), 8(a), HUB-Zone, Women Owned Small Business, Small disadvantaged business, or Small Business). (g) A capability statement that addresses the organization s qualifications and ability to perform as a contractor specifically for the work described in paragraph 5 below. Respondents are also encouraged to provide specific examples (e.g. contract number, point of contact information) of the Contractor s experience providing the same or similar supplies/services to that described in paragraph 5 below. 4. SDVOSB/VOSB respondents. (a) If respondent is VOSB or SDVOSB, respondent is encouraged to provide proof of www.vetbiz.gov certification. In addition, SDVOSB/VOSB Contractors are asked to acknowledge that they understand the limitations on sub-contracting pursuant to FAR 52.219-14, Limitations on Sub-Contracting, which will be included in the impending solicitation. Further SDVOSB/VOSB contractors are asked to respond that they are capable of providing the requested services keeping within the parameters of this clause. (b) Small businesses, including SDVOSB/VOSBS are also advised that per 13 CFR, § 125.6 the prime contractor's limitations on subcontracting are as follows: (1) General. In order to be awarded a full or partial small business set-aside contract with a value greater than $150,000, an 8(a) contract, an SDVOSB contract, a HUBZone contract, a WOSB or EDWOSB contract pursuant to part 127 of this chapter, a small business concern must agree that: (2) In the case of a contract for services (except construction), it will not pay more than 50% of the amount paid by the government to it to firms that are not similarly situated. Any work that a similarly situated subcontractor further subcontracts will count towards the 50% subcontract amount that cannot be exceeded. 5. Please note that if VA s pending market research establishes that two or more VOSB or SDVOSB, who are certified in VA s Vendor Information Pages (VIP) site https://www.vip.vetbiz.gov/, can provide the services, VA will set aside the underlying solicitation per 38 U.S.C. §8127.   6. All Offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) database found at https://www.sam.gov.
