- The Atlanta VA Medical Center anticipates a requirement to provide Temporary Overnight Housing for Male Veterans emergency shelter and food for no longer than 72 hours for 5 Male Veterans authorized by the Department of Veteran Affairs. This will have a performance period of a base year plus four (4) 1 Year Option years and will be Firm Fixed Priced (FFP). The performance will occur at the Contractor s location and Government furnished property will not be provided. Health Care for Homeless Veterans (HCHV) provides case management services to homeless Males Veterans and Facilitates their access to a broad range of medical, substance abuse, mental health, rehabilitative, and employment services, HCHV s goal is to engage homeless males Veterans in supportive services and place them in a safe and secure environment that supports their rehabilitation goals. Care will include, but not be limited to: Mental Health Substance Abuse Treatment Employment Assistance All interested vendors are invited to provide information to contribute to this market survey/sources sought including commercial market information. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONES: Company name, address, phone number, primary contract(s), email address, NAICS code(s), business size, (i.e., small/large) and DUNS Number. Provide a Statement of Capability that demonstrates the offeror s past performance in providing this type of service. Submit Capability Statements via email to cheryl.eason@va,gov by 22 April 2019. Responses should be received no later than 5:00 PM EST. NO CAPABILITY STATEMENTS MAY BE SUBMITTED AFTER 22 APRIL 2019. AT THIS TIME NO SOLICITATION EXISTS.
