5-3 32Statement of Work Diagnostic Specimen Transport Services A.1 SCOPE A.1.1 Description of Service/Work Statement 1) Pickup Diagnostic Specimens from Coatesville VA Medical Center, Laboratory (Building 3, Room125, 1400 Blackhorse Hill Road, Coatsville, PA 19320 and transport to the Philadelphia VA Medical Center Laboratory, 3rd floor, room 138-3 B Accessioning Area, University and Woodland Avenues, Philadelphia, PA 19104 weekdays Monday through Friday. This is a scheduled same day service, one trip per day. Pick up time is between 10:30am and 12:00 pm. Delivery in Philadelphia VAMC is not later than 2:00 pm. 2) Pickup Diagnostic Specimens from Spring City Community Based Outpatient Clinic (11 Independence Drive, Spring City, PA 19475) to transport to Coatesville VA Medical Center Laboratory (Building 3, Room 125) weekdays Monday through Friday. This is a scheduled same day service, one trip per day. Pick-up time is between 10:30 am and 12:00 pm. Specimens must be delivered to Coatesville VA Medical Center no later than 1:00 pm. 3) Pickup Diagnostic Specimens from Delaware County CBOC (4883 West Chester Pike, Newtown Square, PA 19073) and transport to Coatesville VA Medical Center Laboratory (Building 3, Room 125) weekdays, Monday through Friday. Pick-up time is between 10:30 am and 12:00 pm. This is a scheduled same day service Specimens must be delivered to Coatesville VA Medical Center not later than 1:00pm. Courier shall provide secure carrier capable of containing inner packs of biohazard material and meeting DOT requirements. Diagnostic Specimens shall be transported in a manner that preserves the integrity of the samples. Each lot may contain several Diagnostic Specimens in more than one package. A.1.2 Contractor s Personnel The contractor shall provide sufficient personnel who meet all requirements herein and who can be relied upon to perform in strict accordance with the contract. The Chief of Staff will be the final determiner to appropriate qualifications. At all times during performance of this contract, Contractor employees shall conduct themselves in a professional manner, wear clean, neat, and appropriate clothing, and be identified by having company name and/or logo, and employee name attached to their clothing, by name tag or other means. The Contractor shall be responsible for acquiring name tags or badges to meet this requirement. Employee identification shall be worn in clear view above the waist while on the premises of either VA Medical Center. A.1.3 Duty Hours The contractor shall provide a plan to ensure availability of diagnostic transport services for Monday, Tuesday, Wednesday, Thursday, and Friday to ensure arrival at Coatesville VAMC laboratories to the Philadelphia VAMC laboratories by 2:00 pm. A.1.4 Quality Assessment and Improvement The Government may evaluate the quality of professional and administrative services provided, but retains no control over the medical, professional aspects of services rendered (e.g., professional judgments, diagnosis for specific medical treatment), in accordance with Federal Acquisition Regulation (FAR) 37.401(b). The contractor shall have a written quality assurance plan which includes the following: 1) providing documentation that a quality control program is designed and implemented that minimizes patient and public risks and maximizes the quality of services 2) assuring that a qualified individual monitors the provision of services. 3) resolves identified problems and pursues opportunities to improve services. The written plan shall delineate the objectives of the quality assurance activities, scope of the activities, activities for monitoring and evaluation, mechanisms for taking follow-up action and responsibilities for each activity of the quality assurance program. The contractor s Quality Assurance Plan shall be reviewed by the VA Medical Center. Any deficiencies shall be corrected within thirty (30) working days after written notification from the Contracting Officer to assure compliance with VA Quality Management Programs. The contractor shall provide quality reports every six months to include Coatesville VAMC data on timeliness, adverse events, and customer satisfaction. This data shall be provided to the designated Contractor Officers Technical Representative (COTR) March and September of each year, for the life of the contract. It will be reviewed by Coatesville VAMC s Quality Improvement office which reserves the right to require quality reports and action plans at any time during the life of the contract. A.1.5 Services Contractor shall provide all personnel, reliable and serviceable vehicles, supervision, and other required items necessary to furnish scheduled courier/transport services on Weekdays, Monday through Friday to the VA Medical Center, Philadelphia, PA from the VA Medical Center, Coatesville, PA; to Coatesville VA Medical Center from Spring City CBOC; and to Coatesville VA Medical Center from Newtown Square CBOC. Services include the pickup, transport, and delivery of Diagnostic specimens. A.1.6 Description of Duties/Specific Tasks Specimen Preparation The VA Medical Center Laboratory or Nursing personnel will properly prepare and package Diagnostic Specimens for Transport for the contractor. The Diagnostic Specimens are temperature sensitive and shall be transported in such a manner as to ensure the integrity of the Diagnostic Specimens. Pickup and Delivery Pick up: Coatesville - Diagnostic Specimens from Coatesville VA Medical Center, Laboratory (Building 3, Room 125), 1400 Blackhorse Hill Road, Coatesville, Pa 19320. Pickup time is between 10:30 and 12:00 pm. Springfield (Delaware County) Diagnostic Specimens from Delaware County CBOC, 4883 West Chester Pike, Newtown Square, PA 19073. Pickup time is between 10:30 and 12:00 pm. Spring City Diagnostic Specimens from Spring City CBOC, 11 Independence Drive, Spring City, PA 19475. Pickup time is between 10:30 and 12:00 pm. Delivery: Philadelphia - Diagnostic Specimens to the Philadelphia VA Medical Center, Laboratory (3rd Floor, Room 138-3B) Accessioning area, University and Woodland Avenues, Philadelphia, PA 19104. Delivery in Philadelphia VAMC laboratory is no later than 2:00 pm. Coatesville - Diagnostic Specimens to the Coatesville VA Medical Center, Laboratory (Building 3, Room 125), 1400 Blackhorse Hill Road, Coatesville, PA 19320. Delivery to Coatesville VAMC laboratory is no later than 2:00 pm. This is a scheduled same day service, one trip per day on Monday, Tuesday, Wednesday. Thursday and Fridays. A.1.7 Administrative Elements The contractor shall submit monthly invoices for services rendered no later than thirty (30) calendar days following the close of the month in which the services were rendered.. A.1.8 Liability Insurance The contractor agrees to maintain liability insurance in the provision of its professional services. Contractor shall provide a Certificate of Insurance to Coatesville VAMC as evidence of insurance coverage. In the event of litigation/investigation of a claim, the contractor and/or the contractor s personnel shall cooperate fully with the Government authorities and designated officials in the investigation of the claim or preparation for litigation. A.1.9 PERFORMANCE Contractor shall perform the requirements of the contract within the designated time frames while maintaining all sample integrity. A.1.10 QUALIFICATIONS Contractor must have all licenses, permits, and/or certification as required by Federal, State, and local laws and regulations for the transport of Diagnostic Specimens. Drivers must possess appropriate vehicle operator licenses. Contractor shall ensure that all contractor employees directly engaged in the transport of Diagnostic Specimens under this contract receive adequate training regarding DOT C.F.R. 49 Hazardous Material Requirements and OSHA C.F.R. 29 (with particular emphasis on transport of bio-hazardous material and Standard precautions related to blood-borne pathogens). Contractor shall supply evidence of such training (Training certificates or similar). Contractor shall equip courier vehicles with OSHA Standard Precautions Compliance Kit. The kit is a combination personal protection kit and clean-up kit to aid in the clean-up, transportation and disposal of up to 1 full liter of infectious blood or body fluid spills. Kits are readily available at nominal cost from numerous commercial sources. In the performance of contract services, the contractor shall observe the requirements imposed by law, Federal regulation, and VA statutes and policies concerning privacy and/or release of sensitive data/information. The contractor shall ensure that their personnel receive training in and meet these requirements and that confidential and proprietary information shall be released only to those officers and officials of the VA or contactor authorized to receive such information. The contractor must comply with all Department of Veterans Affairs, Health Insurance Portability and Accountability Act (HIPPA), and healthcare industry security standards. Information received from the VA Medical Centers is the property of the U.S. Government. The Contractor must maintain strict security measures. All information (written, oral or electronic) is confidential and falls under the requirements for the Privacy Act of 1974 and HIPPA. The contractor shall not transfer, copy, distribute or disclose said information without direct authorization of Department of Veterans Affairs personnel. A.1.13 EVALUATION FACTORS Awards shall be made to the offeror whose proposal is most advantageous to the Government, this based upon all of the above noted factors being considered. The following factors, in order of importance, will be used in evaluating offers. Award will be based upon the highest point score. Offerors are reminded to address each of these factors in detail. To ensure proper consideration and evaluation of your proposal, you are advised to fully outline your intent to meet or exceed the requirements of the contract.
