- THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. This is not a solicitation for proposals, proposal abstracts, bids, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Responses to the information requested will assist the Government in determining the appropriate acquisition method. The Great Lakes Acquisition Center (GLAC) is soliciting businesses to provide access to a subscription-based patient education television portal or equivalent software for the Adam Benjamin, Jr. (ABJ) Outpatient Clinic, 9301 Madison Street, Crown Point, Indiana, 46307, to be broadcast on monitors throughout the clinic waiting areas. The network subscription will provide customized patient education for all veterans within the outpatient setting. The system provides a VA Solutions Library with lists of multiple video content that would be available online as well as the preloaded tablets with the VA Limited Custom level. It is understood that all Contractors submitting a response to this Sources Sought will have an established business with three (3) years of consecutive experience in performing these types of services at healthcare facilities. The Contractor must also be licensed and authorized to perform services on all the equipment listed. The Contractor shall furnish all labor, materials, tools, components, equipment and supervision for the OEM annual service and testing necessary to complete the required services. The following NAICS Code and Small Business Size Standard has been identified for this requirement: 515120 Television Broadcasting with a Size Standard of $38.5 Million in annual revenue. The contractor will license the use of the patient education television portal or equivalent software to the Adam Benjamin, Jr. CBOC and support the installation, implementation, and customization of the program. The license grant includes the right to make and use a reasonable number of copies of the Software for testing, back-up, archival and disaster recovery purposes, and of the documentation for internal training, support and deployment. The contractor will provide the following solution: Integration of patient education material with health literacy and appropriate patient educational content to meet the needs of the veteran population; Report functionality to enable continual quality improvement and performance tracking; Administrative interface to perform user management, application configuration and auditing; Integrate the network software with the hospital-based television software used for evidence-based patient instructions. The network patient education channel will be an add on to our current television system and is customized for the VA environment to help educate veterans about their care and additional VA programs. This is a new software that will interface with the current cable television to improve patient care and understanding of health diagnosis. It allows the care givers to have one visual source in all areas with educational content Allows care givers to have one visual source ensure patients understand their health diagnosis. Technology that is table integrated application intended to improve the patient experience by providing education regarding patient diagnosis and allows the caregiver to review steps on what to do if a problem arises, assesses the patient s understanding of the plan, gives the patient a written content to take home for review and follow. Interested parties shall provide a brief overview of their firm s capabilities and past experience with projects similar in size and scope of this project. Capability statements shall also include: DUNS number Organization Name and Assumed Name/dba (if applicable) Organization Address Point of Contact (including name, title, addresses, telephone number, fax number and Email address) Business size (small business or other than small business) Socio-economic status (If SDVOSB or VOSB provide SDVOSB or VOSB CVE verification) Organization experience in performing these types of services Copies of applicable certifications and/or accreditations (All information shall be submitted in Word or PDF format, not to exceed 10 pages, including all attachments.) Firms shall identify whether they are a small business or other than small business in accordance with the size standard for NAICS code 515120. If another NAICS code may be applicable, please identify the NAICS in your capability statement along with your corresponding socio-economic status.
