Description
This Sources Sought Notice is for informational and planning purposes only and shall not be construed as a solicitation or as an obligation or commitment by the Government. This notice is intended strictly for Market Research. This is a Request for Information only. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The Department of Veterans Affairs, Dallas VA Medical Center in Dallas, Texas intends to award a Firm-Fixed Price contract for the purchase of Stents. (See attached for a description of items). The Government is conducting a market survey to help determine the availability and technical capability of qualified service-disabled veteran-owned small businesses, veteran-owned small businesses, small businesses, HUBZone small businesses and/or other large businesses capable of serving the needs identified below. This notice of intent is for open market as well as Federal Supply Schedule (FSS) items. If you have these items or similar items on FSS, please respond with your FSS Number. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, women owned small business, HUB Zone, 8(a), small business or large business, relative to NAICS 339112. Responses to this notice will be used by the Government to make appropriate acquisition decisions. A solicitation is not currently available. If a solicitation is issued, it will be announced on Federal Business opportunities website http://www.fbo.gov or GSA E-Buy at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. ***Brand-name or equal accepted. *** Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. ITEM INFORMATION ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 1.00 JB __________________ __________________ Inspection of Fire Damper and Radiant Damper in building 221 as per SOW GRAND TOTAL __________________ Performance Work Statement (PWS) Fire Damper Testing Building 221 1.0 Introduction The VA requires that the Fire Dampers in Building 221 be tested to ensure proper operation as per NFPA and Joint Commission requirements. 2.0 Background This testing of the Fire Dampers and Radiant Dampers are a service that is required periodically to ensure the safety of the patients and staff. The Testing is mandated by NFPA. 3.0 Scope The Vendor must visit site prior to bid in order to be fully acquainted with existing site conditions and scope of work. The contractor responsibility to field verify all existing devices, equipment, measurements, equipment locations and areas provided in scope of work. The Contractor must provide all labor, equipment, materials, tools, testing and supervision for inspection and testing of fire and smoke dampers. The Contractors Technician shall have fire protection testing qualification and certificate. The Contractor shall perform all testing and inspection in accordance with NFPA 80 and NFPA 92 and other NFPA Code. The Contractor Technician shall be HVAC Certified and have NICIA Qualification for Testing of Mechanical Fire Dampers. There is an estimate of 65 Fire Damper and 180 Radiant Damper located with building 221. 4.0 Applicable Directives NFPA 80, 92 and other NFPA Code as it pertains to the Testing of Fire Dampers 5.0 Performance Requirements (Identify what the contractor is required to do (major tasks and subtasks). This contract is for testing only no repairs are to be made to any parts of the system. If issues arise the vendor will notify the COR who will determine the best method of getting the repairs done to continue the testing. 1.1 Provide the schedule of inspection and testing dampers and Technician certificate 1.2 Provide the list of each damper location at each building (VA will provide drawings) 1.3 Full unobstructed access to the damper shall be verified and corrected as required 1.4 The operation of the damper shall verify and test that there is no interference due to rust or bent, misaligned or damaged frame or blades or defective hinges to other moving parts 1.5 The fusible link shall be reinstalled after testing is complete. Replace any damage link with the same size 1.6 Inspection and testing shall be done by qualified technicians within 30 calendar days 1.7 Coordinate with COR for testing since some rooms which are occupied with patients 1.8 All testing in patient rooms are convenience of Government and coordinate with Staff 1.9 Provide a testing report for each building with all test results within 14 days of test completion 6.0 Deliverables (List all data required to manage or monitor contractor performance). Prior to work begin the contractor must provide the Certificate for the Technician preforming the Testing. The Vendor must provide a full report of all Testing performed that show the device location and date of testing and test performed within 14 Days after completion of testing. Performance duration will be 30 days after notice to proceed.
