Description
THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY COMMUNITY-BASED OUTPATIENT CLINIC (CBOC) This is not a solicitation for proposals, proposal abstracts, bids, or quotations.   The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Responses to the information requested will assist the Government in determining the appropriate acquisition method. The Veterans Health Administration Regional Procurement Office West, NCO 17, Medical Sharing Branch I is conducting a market survey to determine the existence of potential businesses of all concerns (e.g., HUBZone, 8(a), SDVOSB, Woman Owned, etc.) with the capability to provide Community-Based Outpatient Clinic (CBOC) service for the parent facility in North Texas VA Healthcare System (NTVAHCS) in Dallas, Texas. Place of Performance: Department of Veterans Affairs North Texas VA Healthcare System Community Based Outpatient Clinic (CBOC) Denton, Texas Qualifications: The Contractor shall provide continuous delivery of primary and preventive medical care services and mental health space for eligible veterans enrolled with the VANTHCS at the Contractor s clinic facility for all assigned patients, in accordance with the terms and conditions stated in the forthcoming solicitation. The Contractor s clinic site shall be located within the city limits in Denton, Texas. The requirement will be firm fixed price, Indefinite Delivery/ Indefinite Quantity contract, at capitated rate per member per month (PMPM) for an estimated 7,700 veterans living or residing in the Denton, TX, area. If this request for information (RFI) leads to a future solicitation, the Government intends to award a firm-fixed price contract. The performance period shall be one (1) base year with four (4) option years. Interested parties shall provide their firm's capabilities statement that support experience with projects similar in scope to this requirement. All information shall be submitted via email in Word or PDF format, not to exceed 10 pages, including all attachments. Each capability statement submittal shall also include the following business information: a. Organization Name b. Organization Address c. Organization DUNS Number d. Point of Contact (including name, title, addresses, telephone number, fax number and Email address) e. Business size under NAICS 621498 (Business Size Standard $20.5M) f. Socio-economic status (IF SDVOSB or VOSB - must be registered in VIP (https://www.vip.vetbiz.va.gov/) g. Contractors must be registered in Systems for Award Management (https://www.sam.gov/SAM/) Firms shall identify whether they are a small business or other than small business in accordance with the size standard for NAICS code 621498.
