Department of Veterans Affairs West Texas VA Healthcare System Statement of Work Quick Inflatable Tent System 519-19-2-6166-0062 Scope of Work: The Contractor shall provide a quick set up external inflatable Hospital tent system during Emergency response. Tent System shall be delivered to the West Texas Veterans Affairs Health Care System (WTVAHCS), 300 Veterans BLVD. Big Spring TX, 79720. The inflatable tent (shelter) system shall be a lightweight durable fire and UV resistant PVC coated fabric (military grade 22oz/sq. yd.) rapid response inflatable tent system, with two (3 x 3 ) VA logos. The tent system shall be inclusive of but not be limited to, the furnishing of all necessary labor, materials, parts, tools, equipment to provide in accordance with the terms, conditions, provisions and specifications contained herein, and in accordance with manufacturer s instructions and specifications. Location of Work: Work will be conducted at the Big Spring location. Big Spring: 300 Veterans Blvd, Big Spring, TX 79720 Contractor Deliverables: Description Quantity/Each 5.0 Ton Air Conditioning (62,000 BTU)/Heat Pump Unit. Color:White. 1 (2) 60-240V 24 LED Light Fixture w/ Protective Case 5 2/4 Wheel Heavy-duty Transport Cart 2 PVC COATED FABRIC Shelter, 6 arches, 2 doors, 20 8 x 40 6 x 10 2 w/master blaster Hose Kit White 840 sq. ft. 1 30- Interior Air Distribution Plenum 30 1 Inner Insulation Sheet, Cotton, for PVC COATED FABRIC for 6 arched shelter 1 Inner Insulation Sheet, Cotton, for PVC COATED FABRIC 5 arched shelter 1 Emergency Preparedness Consultation & Training Service (One day on site training) 1 Shipping Charge 1 Work shall be coordinated with the Project Manager. Safety barriers, signage, and general cleanliness are a priority on this project. Any deviation from safety, or other mandated measures will be cause for training to terminate until these deficiencies are corrected. The contractor shall thoroughly clean inside and outside of daily with contractor s own equipment. The contractor shall adhere to basic VA specifications for all work and VA identification and security policies. Salient Characteristics: The PVC COATED FABRIC is transportable by two people The PVC COATED FABRIC tent can be erected by one person in less than five minutes The tents can be interconnected to each other easily to make a star-shaped configuration as well as other shapes, which is important when connecting with other VAMC PVC COATED FABRIC systems within the Care Region. The PVC COATED FABRIC tents are highly resistant, fire retardant and UV protective PVC coated fabric with the cover and floor mat high frequency welded together to form one piece. Climate resistance: 100 km/h wind and 10 daN/m2 snow load resistance. Comes in white to help reflect the sun and withstand the high temperatures and dry arid weather in West Texas region. The PVC COATED FABRIC 5A allows for 603 sq.ft. of floor space and the PVC COATED FABRIC 6A WL allows for 840 sq.ft. Lengths and width PVC COATED FABRIC 5 - 32 8 and PVC COATED FABRIC 6 - 40 6 with heights of 9 2 and 10 2 respectively. Ongoing of complete coverage of customer care post-sale and training. Proven track record with multiple VA systems of system design, implementation and onsite training are unique factors. Material: Material purchased by the contractor must meet the specifications and submittals shall be approved prior to purchasing the material. Requirements: The contractor shall provide all applicable safety signage and barricades applicable to OSHA and VHA regulations during training. Contractor shall comply with VHA DIRECTIVE 2004-012 and all VA Center Policy Memorandums, including but not limited to the following: Protection of patients, visitors, and employees from traumatic injury, as well as occupational and facility associated infections. (b) Compliance with OSHA and State construction safety regulations. (c) Compliance with EPA and State environmental regulations. Applicable Standards and Acceptance Criteria: Meet Salient Characteristics Base bid shall include all labor and materials to execute the above scope of work. INFORMATION SECURITY AND PRIVACY The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract. Consistent with the requirements of 38 U.S.C. 5725, a contract may require access to sensitive personal information. If so, the contractor is liable to VA for liquidated damages in the event of a data breach or privacy incident involving any SPI the contractor/subcontractor processes or maintains under this contract. However, it is the policy of VA to forgo collection of liquidated damages in the event the contractor provides payment of actual damages in an amount determined to be adequate by the agency.
