Page 1 of 2 This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for quote (RFQ) and it does not obligate the Government to award a contract. Request for a solicitation will not receive a response. The purpose of this sources sought announcement is for market research purposes; to make the appropriate acquisition decisions; and to gain knowledge of potential qualified sources and their Small Business Administration (SBA) size classification in relation to North American Industry Classification System (NAICS) 811198 All Other Automotive Repair & Maintenance. The Department of Veterans Affairs, Network Contracting Office (NCO) 22 is seeking sources to provide the following professional services at the VA Loma Linda Healthcare System (VALLHCS): Streetsweeper Quarterly Maintenance The VA will require the maintenance to be conducted on the facility property at a quarterly interval such as May, September, January, May for the year 2019 2020. The vendor will conduct all maintenance on the sweeper during normal business hours between 7:00AM 4:30PM Monday through Friday with proper coordination with the Grounds supervisor and the contracting officer representative. All maintenance will be completed in accordance with the manufacturer recommendations for the appropriate model: M636 Green Machine. Serial #: 20121326. Major Duties and tasks include, but are not limited to: Vendor will conduct all inspections, diagnostics, lubrications, and adjustments of the machine s components to include: air cleaner, fuel filter, van fan filter, seals, battery power, hydraulic fluids, electrical systems, drive chain/belts, debris pickup, squeegees, ignition parts, and oil/filter. The pm check list for the applicable model is as follows to inspect, replace, or adjust the following items: Air cleaner, fuel filter, hydraulic fluid, hydraulic filter, oil, oil filter, antifreeze, misc. lubricants, skirt maintenance, seal maintenance, hopper seals, and drive belts. The vendor will accept responsibility of all trash and waster after the completion of each service. All waste will be disposed of in the applicable manner offsite after each service. Proper documentation will be maintained by the vendor and copies of each maintenance will be provided to the medical center at the closure of each service for accurate record keeping. Any repairs, replacements, or other non-covered maintenance items will require an appropriate quote and authorization to complete the work. The vendor will furnish all applicable warranty coverages to the medical center so that the facility can track and maintain proper warranty information for the applicable equipment. Streetsweeper quarterly maintenance duties include, but are not limited to: The Contractor will furnish all materials, equipment, tools, supervision, and personnel necessary to accomplish the remediation actions as indicated on the attached contractual documents. All employees of the vendor shall comply with the VA security management program and obtain permission of the VA police, be identified by project and employer, and restricted from unauthorized access. Prior to commencing work, the vendor shall provide proof that an OSHA certified competent person (CP) (29 CFR 1926.20(b)(2) will maintain a presence at the work site whenever the general or subcontractors are present. Offeror must possess a 30-hour OSHA safety card. (www.osha.gov) Prior to the commencement of work, the contractor shall conduct a site walk with the VA COR and VA Grounds Supervisor. Contractor will provide all documentation per Specifications and validated by the submittal registry to the COR prior to work. All interested offerors who can provide this service shall submit a written response to this notice and include the following: Company name, Address, Point of contact, Phone, fax, and email, DUNS number, Cage Code, Tax ID Number, Type of small business, e.g. Services Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), 8(a), HUBZone, Women Owned Small Business (WOSB), Small Disadvantaged Business, or Small Business [NOTE: If you are a SDVOSB or VOSB, you must submit your certification from the Center for Verification and Evaluation (CVE) (https://www.vip.vetbiz.gov/)], Provide a copy of your company s current Representations and Certifications from www.sam.gov, Provide evidence your firm can supply the services for this Request for Information (RFI). Firms shall provide the following information on current and/or previous contracts in which you have provided this service. Furthermore, the Government may reach out to your listed point of contact to verify information provided in your response: Contract number Facility s Name and Address Point of contact, to include name, title, phone number, and email of the facility in which you have contracted with. Must complete and sign the following Word Document attached to this notice: S02 QUALIFICATION LIST STREETSWEEPER MAINTENANCE If possible, provide an estimate rate for these services on a per procedure basis. All responses must be received no later than May 1, 2019, 12:00 PM Pacific Daylight Time (PDT).
