Responses to this notice are not offers and will not be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this Sources Sought. All information received in response to this RFI that is marked Proprietary will be handled accordingly. Responses to the Sources Sought will not be returned. At this time, questions concerning the composition and requirements for a future Request for Quote will not be entertained. Subject: The Department of Veterans Affairs is conducting market research in order to provide a service agreement for the Hershel Woody Williams.VA Medical Center. The government anticipates awarding a Firm Fixed Price award contract that will provide a service agreement for Chemdaq Monitoring Systems see below attachment. OBJECTIVE: See attachment. RESPONSE INSTRUCTIONS: Name and Address of Company. Any small business designation as certified by the Small Business Administration. A through description of experience in managing support requirements of this nature. 5.0 CONTACT INFORMATION: Any questions related to this Sources Sought shall be directed to mark Smith at Mark.Smith25@va.gov All information regarding Capabilities Statements or any other proprietary information relative to this Sources Sought shall be submitted via email to Mark.Smith25@va.gov no later than 3:00 pm EST on 5/02/2019 ATTACHMENT 1 STATEMENT OF WORK Subject: Statement of Work: Vendor will provide a Sensor exchange program for toxic Gases used in Sterile Processing Service, GI Suite and Dialysis for a year. The sensors monitor Hydrogen Peroxide, and Peracetic Acid. The monitoring system is ChemDaq and the sensors are ST1-PSXP1 for PAA and ST1-HSXP1 for H2o2. Vendor will calibrate the sensor modules under laboratory conditions prior to shipment to the customer site, via UPS. Vendor will furnish all Calibration Certificates for our records. Vendor will pay for the return shipment for used Sensors. To: Huntington VA Medical Center Services, ATTN: Melissa Adkins, Chief of Logistics, Huntington, WV 25704 Per your request the following Statement of Work submitted for your approval: Contract Renewal Date: July 1, 2019. Work/Service to be completed: Sensor Exchange Program 2.
