Description
COMBINED SYNOPSIS SOLICITATION (i) This is a combined synopsis/solicitation for Brand Name Medline Biocon 700 Bladder Scanners for the Philadelphia VA Medical Center, as prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. A firm-fixed price purchase order is anticipated. (ii) The solicitation number is 36C24419Q0625 and is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. (iv) This solicitation is set aside 100% for Service-Disabled Veteran-Owned Small Businesses and the associated NAICS 339112 code has a small business size standard of 1000. (v) Contract Line Items (CLINS): Medline Biocon 700 Bladder Scanners. B.2 PRICE/COST SCHEDULE ITEM INFORMATION 0001 Scanner, Bladder, Biocon 700 W/Printer 4EA Cost$ ________________ 0002 Stand, MOB, W/Basket 4EA Cost$ ________________ B.3 DELIVERY SCHEDULE Delivery: Philadelphia VA Medical Center, 3900 Woodland Avenue Philadelphia, PA 19104 Statement of SON Bladder Scanner I. Scope of Work The Philadelphia VA Medical Center, located at 3900 Woodland Avenue, Philadelphia, PA 19104, is in need of brand name Medline BioCon 700 with rolling carts as designated in Section III below. These bladder scanners use real time mode pre- scan aiming with touch screen controls and meet the needs for the Philadelphia VAMC. Medline previously provided a demonstration unit for inspection and trial within 10 days of closure of the announcement at no charge or commitment from the Philadelphia VAMC. Philadelphia VAMC has used the Verathon BVI 3000 bladder scanner exclusively throughout the medical center and community living center but has since replaced all bladder scanners with the Medline BioCon 700 on power stand. II. Contractor-Furnished Equipment and Supplies The contractor shall provide all resources necessary to support demonstration and trial use of devices using offering in-service, education, and a bladder scanner assessment of facility if necessary to ensure all end users are educated on using the bladder scanner. III. Performance Requirements Requirements The vendor shall provide all labor, parts, travel, tools, and equipment to perform all inspections, testing, repair, replacement, and maintenance as required at a predetermined cost when out of warranty and necessary, to maintain full functionality of the devices. The warranty period should be a minimum of five years (on site) and include up to two preventive maintenance services per scanner. All bladder scanners provided by the vendor shall meet the minimum specifications as noted below. Each manufacturer is responsible to insure their product meets or exceeds these requirements. Clarification of requirements follows: Touch screen controls Real time mode pre-scan aiming Post-scan aiming guidance via console Confirms peak volume via additional scans Accuracy range of + (15% + 15mL) Scan from either side of patient Distinct scan modes for men, women, and small children   One button data capture USB and SD card ports Automated volume calculations Scan time of less than 3-5 seconds Ability to be mounted on cart for mobility Premium cart quiet, durable, with lockable wheels FDA approved and CE certified medical device Models must be able to be repaired in the United States Owner Manual The contractor shall provide one (1) copy of an owner s manual for each device. The contractor shall provide five (5) copies of the repair manuals to Engineering for use by the designated section responsible for inspection, maintenance and repair. Warranty The contractor shall provide at least a five-year manufacturer s warranty on parts and labor related to manufacturer defects and drop protection. The warranty shall include all travel and shipping costs associated with any warranty repair. The drop protection shall include one no-charge probe repair or probe replacement and be limited to one repair for each scanner. B. GOVERNMENT FURNISHED PROPERTY: The government will furnish no property to the contractor. CLAUSES: (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and the following clauses AND instructions are added as addenda: CLAUSES: 52.214-21 Descriptive Literature (APR 2002) 852-219-10 VA Notice of Total Service-Disabled Veteran-Owned Small Business Set-Aside (a) Gray market items are Original Equipment Manufacturer s (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers.   No remanufactures or gray market items will be acceptable.   (b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM.   All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. (c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract.   Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to  replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.   (ix) Evaluation of this requirement will be based on PRICE ONLY (AUTHORIZED DISTRIUBTOR LETTERS FROM THE MFG MUST BE INCLUDED) (x) Offerors are advised to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer if has not been completed on SAM.gov.
