Description
STATEMENT OF WORK VETERANS AFFAIRS MEDICAL CENTER, PHILADELPHIA, PA REQUEST FOR QUOTE FOLLOWED BY TENTATIVE STATEMENT OF WORK Veterans Health Affairs, Network Contracting Office 4, is issuing this Request for Quote (RFQ) in order to identify capable firms to provide Custom Fit Lead Aprons for the Philadelphia, PA VA Medical Center. Responses must be submitted by May6th, 2019 by 5:00 PM Eastern Standard Time (EST). Submit responses to the information requested above via email to rashaun.thomas@va.gov. All SDVOSB and VOSB firms that respond shall include proof of CVE certification via www.vip.vetbiz.gov. All small business firms that respond shall include proof of small business status via their Representations and Certifications in accordance with (FAR 4.1102 Policy). All firms that respond must be registered in the System for Award Management via https://sam.gov/SAM/. All firms who respond must submit their Duns & Bradstreet number via https://www.dnb.com/. Any firms holding an GSA schedule contract from https://www.ebuy.gsa.gov/advantage/ebuy/start_page.do must submit the GSA contract number. All firms who respond must submit their tax ID number. While SDVOSB/VOSB contractors are preferred, all capable contractors are welcome to respond to this source sought notice for market research purposes, and shall include as part of their response a brief capability statement that covers the information in the following tentative statement of work. The results of this market research will assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., socioeconomic set-aside, full and open competition, etc.). VA assumes no responsibility for any costs incurred associated with the preparation of responses. This RFQ will accept any recommendations for Product Service Code or NACIS codes. ALL OFFERORS SHALL USE THE PRICE SCHEDULE LOCATED IN SECTION B TO QUOTE THE PRICE FOR EACH LINE ITEM. OTHER FORMATS ARE NOT ACCEPTABLE. Each offeror MUST fill out the information requested in pricing schedule for each line item or your quote will be deemed non-responsive. EVALUATION FACTORS SOLICITATION NOTICE OF TIERED EVALUATIONS This procurement is set-aside based on an order of priority as established in 38 U.S.C. 8127 TIERED EVLAUATIONS INCLUDING SMALL BUSINESS CONCERNS This solicitation is being issued as a tiered evaluation for SDVOSB concerns, or in the alterative, a tiered evaluation for VOSB concerns, or in the alternative, a set-aside for other small business concerns with HUBZone small business concerns and 8(a) participants having priority. If award cannot be made, the solicitation will be cancelled, and the requirement resolicited END The evaluation of offers received in response to the solicitation will use a tiered or cascading order of precedence. Tiered evaluation of offers, also known as cascading evaluation of offers, is a procedure used in negotiated acquisitions when market research is inconclusive for justifying limiting competition to small business concerns or sub-categories of small business concerns. The contracting officer (1) Solicits offers from both small and other than small business concerns that will be evaluated in the following tier order: (a) service-disabled veteran-owned small business (SDVOSB); (b) veteran-owned small business (VOSB); (c) all other small business concerns; (d) other than small business. (2) If an award or a sufficient number of awards cannot be made at the first tier, evaluation of offers will proceed at the next lower tier until an award or a sufficient number of awards can be made. TENTATIVE STATEMENT OF WORK All aprons shall be in compliance with CMC VAMC s AS Low As Reasonably Achievable (ALARA) program, OSHA, Joint Commission and VHA requirements, Radiation Safety supplies radiation protection devices to employees receiving radiation exposure while performing daily clinical applications requiring x-ray radiation. The involved Services include but, are not limited to: Radiology, Cardiac Catheterization Lab, Gastrointestinal (GI), Dental, Operating Room (OR), and Research. 1. A) Each lead apron shall be fitted to the person for use. e.g. (The vendor shall come to this facility at a time specified by the facility to measure each person to be fitted for lead aprons.) Times will be Monday thru Friday, between 0800 and 1600. The Vendor may also bring samples of lead aprons in the following general sizes: small, medium, large, and X-large for purposes of fitting the correct size. 2. All Lead Aprons shall be of the type equal to the following: Vest/ Kilt, Full wrap around or full frontal based on the use. 3. All Lead Aprons shall be made of lead equivalent to 0.5mm lead in the front and 0.35mm back. (e.g. 0.25 mm over lapping in the front equals 0.5mm coverage.) 4. Thyroid shields will be attached to the Lead Apron, 0.5mm lead. 5. Shoulder lift attachment will be proved upon request. 6. Attached waist belt will be provided upon request. 7. All name tags shall be of the Velcro type above the left breast pocket. No other embroidery or badges, etc. is required. 8. The color of the lead apron will be determined at the fitting. Product acceptance, testing and delivery: All Lead Aprons provided by the vendor shall go through quality control testing by CMC VA Medical Center Radiation Safety staff prior to being accepted by this facility. Quality control testing will be both qualitative and quantitative (x-ray tested). All product must pass this testing or be returned to the vendor at no cost to this facility. Final product (Lead Aprons) shall be delivered not more than 45 days post initial fitting/ measurement.
