The Veterans Administration Commodities & Services Acquisition Service, Denver Logistics Center Request for Information for Commercial Item Purchase THIS SOURCES SOUGHT IS NOT A REQUEST FOR PROPOSAL This notice is a market research tool being used to determine potential and eligible business firms that are capable of providing the products described herein prior to determining the method of acquisition. BACKGROUND The Veteran Administration (VA), Commodities & Services Acquisition Service manages holistic supply chain management for the VA National Audiology, Prosthetics, and Telehealth Programs and supports VA and other Government agencies with professional acquisition and logistical services. We seek information on vendors that are capable of supplying products that are Food and Drug Administration (FDA) Class II approved: Auditory Osseointegrated Implants (AOI). This announcement is not a Request for Quotes (RFQ) and does not commit the Government to award a contract now or in the future. No solicitation is available at this time. Additionally, the Government is not obligated to and will not pay for any information received from potential sources as a result of this notice. Purpose: The Auditory Osseointegrated Implant devices provide auditory signal through bone conduction. These devices have been identified as a preferred product for consolidated contracting; consolidation will result in increased user uniformity and quality products for our Veterans. Request for Information: The results of this Sources Sought notice will be utilized to determine eligible firms, and product lines, prior to determining the method of acquisition and issuance of a solicitation. This requirement is assigned a NAICS code of 339113, Surgical Appliance and Supplies Manufacturing. As a result of this RFI notice, the VA CSAS may conduct a competitive procurement and subsequently award a contract. Your responses to the information requested will assist the Government in determining the appropriate acquisition method. A determination by the Government not to compete this requirement based upon responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement. In order to receive an award, contractors must have valid registration and certification in the System for Award Management (SAM). Interested business organizations that believe they are capable of meeting the requirement should submit electronic copies of their responses to the questions below as well as any applicable documentation. Responses should include the following information: Company information that will provide product, to include: Name, DUNS, Address, Phone Number, and Email Address. 2. Name of company that will manufacture the product, and location of manufacturing State or Country. 3. Spec sheets and examples of products that meet the description above. 4. Typical lead time to deliver the product after receipt of an order. 5. What is the minimum data requirements necessary to register device for FDA recall purposes? Is DOB necessary if pediatric or adult is noted on form? Please provide company FDA Registration Number, and current copies of FDA approved vendor AOI order forms. Can you accept data via EDI format, X12? Do you have an online portal for registering devices? How often do new items and/or software updates receive FDA approval? Any other relevant information that is not listed above which the Government should consider in finalizing its market research. Technical Specifications: Any proprietary information should be so marked. Only electronic statements will be accepted, and faxes will not be allowed.
