- This Notice of Intent to Sole Source is not a request for competitive proposals. No solicitations documents are available. The Department of Veterans of Affairs, James J. Peters Medical Center in Bronx, NY intends to place a service procurement for PET/CT 6e-68 Phantoms to Siemens Medical Solutions USA, Inc. under other than full and open competition basis with the original end manufacturer of the PET/CT Scanner Biograph System: Siemens Medical Solutions USA, Inc., 40 Liberty Blvd, Malvern, PA 19355-1418. The purchase is for services necessary to supply, deliver, and remove Ge-68 phantom sources for the PET/CT scanner. Ge-68 phantoms provide 2D and 3D normalization and in test images of PET and PET/CT systems. This is a recurring service and the contract will have a 12-month base period plus two (2) Option Years to be exercised at the discretion of the Government. The scope of work will require, but is not limited to, the following: Furnish all necessary labor, equipment, tools, and materials to uniformly fill PET/CT Phantom in the casts and test the Phantoms at the Contractor facility prior to delivery. Deliver new phantoms and pick-up expired isotopes for Ge-68 phantoms. Orders placed by 5PM EST shall be required by 6:30AM EST the following work day, unless otherwise scheduled. The Ge-68 Phantom must be provided by a board-certified Radiopharmacist and delivered to the Nuclear Medicine Department at the James J. Peters VA Medical Center located at 130 West Kingsbridge Road, Bronx, NY 10468. The contractor must be licensed by the Nuclear Regulatory Commission and be regularly established in the business of providing radiopharmaceuticals. Additionally, the contractor shall meet the Department of Transportation, NRC, NHPP, FDA, OSHA, USP 797, VA and all other agency rules and regulations (Federal and State). Contractors shall provide copies of licenses and certifications along with proposals. The contactor shall abide by the Title Code of Federal Regulations (CFR 10). Personnel assigned by the contractor to perform services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. A Safety Customer Advisory Notice from the Siemens Medical Solutions USA, Inc. states that utilizing a phantom other than the Siemens solid Germanium-68 phantom may impact the Calibration Factor values generated during PET Quality Control procedure. Depending on the type of uniform phantom utilized to perform PET Quality Control, the PET Calibration Factor may be biased due to the system using a specific mu-value of 0.1cm-1 which is biased on the Siemens solid Germanium-68 phantom. This procurement is being conducted in accordance with FAR Part 6.302-1 and 13.106-1(b)(2), "soliciting from a single source. The NAICS code is 334517 and has a size standard of 1000 employees. This notice of intent is not a request for competitive quotes. However, interested parties may identify their interest and capability to respond to the requirement and submit Capability Statements in writing (via email) no later than 10:00AM EST, May 16, 2019 to: Danelle.Wassink@va.gov. Responses received may be evaluated; however, a determination by the Government not to compete the proposed procurement based upon responses received to this notice is solely within the discretion of the Government. However, responses to this notice will not be considered adequate responses to a solicitation. Determination by the Government not to compete the proposed procurement based upon responses received to this notice is solely within the discretion of the Government.
