STATEMENT OF WORK (SOW) as of 5/07/19 Contract Number: Task Order Number: IFCAP Tracking Number: Follow-on to Contract and Task Order Number: Contracting Officer s Representative (COR): Name: Dr. Wiam Khadra Section: Cardiothoracic Surgery Address: 3495 Bailey Avenue, Buffalo, NY 14215 Phone Number: 716-862-8728 Fax Number: 716-862-8709 E-Mail Address: Wiam.Khadra@va.gov Contract Title: VA WNYHS procurement of Terumo Blood Parameter Monitoring System for the Buffalo VA Medical Center Operating Room. Background: The Buffalo VA Medical Center requires two (2) new Blood Parameter Monitoring Systems for the Operating Room. The new Blood Parameter Monitoring Systems will be replacing older units. The new Blood Parameter Monitoring Systems are needed to manage patient Hemostasis and gases on a continuous base during Bypass period. Scope: The Buffalo VAMC is seeking to purchase two (2) new Blood Parameter Monitoring Systems that meet the clinical, functional, and performance specifications specified in this Statement of Work (SOW). The Buffalo VAMC shall only consider new, latestgeneration Blood Parameter Monitoring Systems. Refurbished or remanufactured will not meet the needs of the Buffalo VAMC. All proposals must be reviewed by clinical staff prior to award for acceptance. Specific Technical Elements: The proposed Blood Parameter Monitoring Systems shall meet the following Buffalo VA Medical Center technical requirements. The proposed CDI Blood Parameter Monitoring Systems shall be a Terumo Blood Parameter Monitoring System 550 or equivalent. The proposed CDI Blood Parameter Monitoring System shall provide a one-year warranty. The proposed Blood Parameter Monitoring System must use optical fluorescence technology to measure pH, pCO2, pO2, and K+ in blood. The proposed Blood Parameter Monitoring System must include a shunt sensor with four microsensors to measure pH, pCO2, pO2, and K+ and a thermistor to measure temperature. The proposed Blood Parameter Monitoring System must provide measurements of pH, pCO2 and pO2 every second during use. The proposed Blood Parameter Monitoring System must provide measurements of K+ every 6 seconds during use. The proposed Blood Parameter Monitoring System must provide measurements of SO2, Hct, and Hgb every 18 milliseconds during use. The proposed Blood Parameter Monitoring System must include a continuous display of the measurements of pH, pCO2, pO2, and K+ in blood. The offering vendor shall provide two copies of the operator s manual for the proposed Blood Parameter Monitoring System. The offering vendor shall provide any other operator (End-user) training media with the purchase of the Blood Parameter Monitoring System. The offering vendor shall provide a listing of all Recalls, Safety Alerts and Field Engineering Change notices pertaining to the proposed Blood Parameter Monitoring System over the past 24 months. A standard 1-year warranty shall be included with the bid proposal. Other warranty options may be detailed (offering vendors not required to do so) but should only be listed as potential options in the bid proposal. Place of Performance: The place of performance for this contract is the Buffalo VAMC.
