- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 in conjunction with FAR Part 13 Simplified Acquisition Procedures and supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. This combined synopsis/solicitation is issued as a request for quote (RFQ) 36C26219Q0693. Submit written offers in accordance with Addendum to 52.212-1, Instruction to Offerors of this solicitation. Oral offers will not be accepted. All firms or individuals responding must be registered with the System for Award Management (SAM) (https://www.sam.gov). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2019-02TA. North American Industrial Classification Standard (NAICS) 238220, with a size standard of $15.0 Million apply to this procurement. The Government is seeking a Contractor to furnish all labor, personnel, materials, supplies, parts, and travel expenses necessary to provide hospital-wide interstitial. These services shall be in accordance with state, Federal, and local policies, laws and regulations. Place of Performance: VA Loma Linda Healthcare System 11201 Benton Street, Loma Linda, CA 92357. Period of Performance: Anticipated to begin end of May for up to approximately 45 days upon award. This is a 100% total service-disabled veteran-owned small business (SDVOSB) set-aside. Provisions / Clauses: The following provisions / clauses: 52.212-1, Instructions to Offerors Commercial, 52.212-2, Evaluation Commercial Items, 52.212.-3, Offeror Representations and Certifications Commercial Items, 52.212-4, Contract Terms and Conditions - Commercial Items, and 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items are applicable to this acquisition. Please see the attached for the Statement of Work (SOW), and other pertinent information relative to this solicitation offerors shall review and be aware of. Basis of Award: Award will be made to the responsible offeror whose offer conforming to the Statement of Work (attached) and the specialized experienced (listed below) will be most advantageous to the Government. The Government s intent is to award without discussions. Lowest price is the only factor that will be used to evaluate offers (see FAR Provision 52.212-2). Award will be made to the lowest priced offer that is determined to be reasonable. After making selection of the lowest price offeror, the Contracting Officer will determine if the selected offeror is responsible. Contractor and/or contractor s employee(s) performing on this contract shall have the technical knowledge, experience, and OSHA certifications to perform this service. Submitting Offer: Offeror(s) are reminded to thoroughly review the solicitation in its entirety and shall submit their offer using the RFQ Animal Anesthesia Equipment Service Part II and other files found in the attachment files of this combined synopsis/solicitation. Offeror is advised the Government may be unable to receive other types of electronic files (e.g. compressed or zip files) or files larger than ten (10) megabytes (MB). It is the offeror s responsibility to ensure all required documents are included and completed as required by this solicitation.
