Services: Recover A-Frame Awning at Building 2 & 42 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes only the solicitation; quotes are being requested and a written solicitation will not be issued. The (36C24819Q0778) is being issued as a request for quotation (RFQ). SITE VISIT:   The site visit will be held this Friday, 5/24/2019, at 10:00AM EST at the James A. Haley Hospital, 13000 Bruce B. Down Blvd (138) Tampa, FL 33612. Contractors are required to contact Serena Jordan at 813 972-7591 or serena.jordan@va.gov to sign up for the site visit. The site visit is highly encouraged and expected prior to submission of quote. VENDOR QUESTIONS:   All questions regarding this RFQ must be in writing and may be sent by e mail to serena.jordan@va.gov.  Questions must be received no later than Tuesday, 5/28/2019 by 10:00 AM EST.   No further questions will be accepted after that date and time.     ANSWERS TO VENDOR QUESTIONS:   Answers to vendor questions will be posted on FBO. You are reminded that representatives from your company SHALL NOT contact any James A. Haley Veteran s Hospital, Tampa, Florida staff to discuss this RFQ during this RFQ process. All questions and concerns regarding this RFQ shall be directed to Serena Jordan, the Contracting Officer, serena.jordan@va.gov. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-101 effective 10/26/2018. This requirement is a 100% Service Disabled Veteran Owned Small Business (SDVOSB) set-aside. In order for a quote to be eligible for award, the quoters submitting a quote shall be certified in Veterans Information Page (VIP VetBiz) http://www.vetbiz.gov/by the closing date of the solicitation. VIP VetBiz will be checked to verify the quoters SDVOSB certification before any evaluation is conducted. The NAICS code for this requirement is 238390 small business size standards of $15.0 mil. Description of requirements for the items to be acquired: The Contractor shall be responsible for removing existing fabric and provide and install approximately 3,000 sq. ft. of new canvas awning covers over existing A-Frame structure near Building 2 and 42. This will also include new metal structure over one window on building 42 located at the James A. Haley Veterans Hospital (JAHVH), 13000 Bruce B. Downs Blvd., Tampa Florida in accordance with (Attachment 3)- the Statement of Work (SOW). The following line item pertain to the description as stated: See (Attachment 1) - Price/Cost Schedule and (Attachment 2) Delivery Schedule. Performance Period: 45 days after contract award. All services are to be delivered to the James A. Haley Veterans Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL 33612. As a part of delivery and acceptance the FOB point is destination. The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and offers must fill out (Attachment 1) Price/Cost schedule. Offerors must submit their technical approach that pertains to the SOW. The provision at 52.212-2, Evaluation -- Commercial Items, will be used. See (Attachment 4) basis for award. To be eligible for award offerors must be registered in System for Award Management (SAM) http://www.sam.gov in accordance with the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items.
