Description
SOURCES SOUGHT ANNOUNCEMENT: The Philadelphia VA Medical Center, 3900 Woodland Avenue, Philadelphia, PA 19104, is conducting a market survey to identify potential sources to provide Wheelchair Van Transportation Services to its veteran beneficiaries throughout the Philadelphia VA Medical Center service area. This announcement is issued for informational planning purposes only and is not a request for quote or announcement of a solicitation. Submitting information for this announcement is voluntary and VA assumes no responsibility for any costs incurred associated with the preparation of responses submitted as a result of this notice. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. The purpose of this market research is to identify capable sources that are both experienced and qualified to provide Wheelchair Van Transportation Services including all vehicles, personnel, management, supplies, and equipment necessary to cover wheelchair van transport services. The contractor shall have available the appropriate number of vehicles to provide services in a timely manner as outline by the contract. Contractor agrees to provide non-emergent wheelchair van services, 24 hours per day, 7 days per week for eligible veterans of the Philadelphia VA Medical Center, Community Living Center, and its Community Based Outreach Clinics located in Camden, Gloucester, Horsham, and Burlington counties. Care will be provided according to the orders of the attending physician. Contractor shall provide patient hand-off services for all veterans. The contractor shall ensure that the veteran is attended by an authorized responsible party at all times. At no time shall the contractor leave the veteran unattended. Contractor shall ensure proper loading and unloading techniques are followed at all times. Patients in motorized scooters must transport to a vehicle seat. The North American Industry Classification System Code (NAICS) is 485991 Special Needs Transportation, and the small business size standard is $15 Million for this type of effort. Based on this information, please indicate whether your company would be a large or small business; and provide the average amount of sales for the last three (3) years or whether you have a socio-economic designation. Capability Statement and Documentation: Companies are invited to provide their capabilities, experience, and knowledge in providing Wheelchair Van Transportation Services. Sources must provide the following information: 1. Company name, address, telephone number, website, DUNS number, CAGE code, business type, socio-economic status (e.g., SDVOSB, VOSB, Woman-Owned, Disadvantaged Small Business, 8(a), general small business, large, business, etc.), SAM registration expiration date, and person of contact (including telephone number and email address). 2. Indicate and explain your capability of providing this service including maintaining any required security clearances and/or additional licensing. Include the states that you are licensed to provide wheelchair van transport services in. 3. Provide a summary of the type of transportation equipment that will be utilized and any relevant licensing. 4. Indicate your company s size under NAICS 485991. 5. Please indicate any government contracts held including government agency, contract number, dollar value, period of performance, and government contact information. In addition to a Capability Statement which includes the above information, submissions can include specifications, brochures, etc. as attachments. Information regarding your company s capacity and capability to provide such services are to be submitted electronically on or before Friday, 06/14/19 by 12:00 noon (EST) addressed to Cynthia Grygier, Contracting Officer cynthia.grygier@va.gov.
