Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this acquisition is NNMC-19-8868. The solicitation is issued as a request for quotation (RFQ) using FAR Part 12 Commercial Items and FAR Part 13 Simplified Acquisition Procedures. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-02, effective May 6, 2019. This is a ISBEE set aside. The North American Industry Classification System (NAICS) Code for this acquisition is 611620 - School and Recreation Instruction with a business size standard of $7.5 Million. The Contract Line Items (CLINs) and pricing structure are as shown below. CLIN 1: One (2)-day Camp, Gallup Service Unit, Gallup, New Mexico CLIN 2: Three (2)-day Camps, Kayenta Service Unit, Kayenta Arizona CIN 3: Three (2)-day Camps, Shiprock Service Unit, Shiprock, New Mexico The Navajo Area Indian Health Service, Northern Navajo Medical Center has a requirement for seven 2-day "Just Move It" Youth Running and Fitness Camps at sites across the Navajo Nation during Summer 2019. See attached statement of work for additional information. The provision at 52.212-1, Instructions to Offerors-Commercial (OCT 2018), applies to this acquisition. 52.212-2, Evaluation-Commercial Items, the award will be made to the lowest priced technically acceptable offeror. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items (OCT 2018), with its offer. FAR 52.212-4, Contract Terms and Conditions-Commercial Items (OCT 2018), applies to this acquisition. 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items (May 2019), applies to this acquisition.
