Combined Synopsis/Solicitation Solicitation 36C24819Q0688 Page 3 of 3 (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6,as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are a being requested and a written solicitation will not be issued. The Department of Veterans Affairs, James A Haley Veteran s Hospital, 13000 Bruce B Downs, Tampa Florida 33612 has a need to procure a Services for Inspection, preventative maintenance treatment, Emergency treatments, training and eradication for Bed Bugs for Veterans lining/working/visiting location on the Statement of Work (SOW). (ii) The combined synopsis solicitation number is issued as Request For Quotation (RFQ) # 36C24819Q0688. (iii) This combined synopsis solicitation RFQ and the provisions and clauses incorporated are those in effect through Federal Acquisition Circular Number/Effective Date 2005-92-1 / 12-19-2016 (iv) The combined synopsis solicitation is 100% Service Disabled Veteran Owned Small Business Concern Set-Aside. The NAICS is 561710 Exterminating and Pest Control Services Business size standard is $11 Million. (v) This combined synopsis solicitation is for the purchase of Exterminating and Pest Control Services. Vendors shall use Pricing and Cost Schedule, which includes all contract line item numbers, quantities and units of issue, to fill in all unit prices and total amount blocks. The Government reserves the right to make no award at all.
