This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufactures or gray market items will be acceptable. Offeror shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items. Offerors providing an or equal product(s) must adhere to the terms in FAR 52.211-6. If an item in this solicitation is identified as brand name or equal, the purchase description reflects the characteristics and level of quality that will satisfy the Government s needs. The salient physical, functional, or performance characteristics that equal products must meet are specified in the solicitation. Wheeling Street, Aurora, CO 80045. SCOPE The Contractor shall deliver all items listed in section 2.5. All items must meet all salient characteristics defined in section 2.5. The Contractor shall provide onsite training for staff per section 5. 2.4. The Contractor shall furnish all personnel, supplies, and equipment required for delivery. 2.5 SALIENT CHARACTERISTICS 2.5.1. Extremity Trainer, Leg and Arm/Upper Body Quantity: One (1) Each Basis of Design: MOTOmed Muvi, RECK 300.000 General Functionality and Utilization The system must be specifically designed and constructed for onsite, clinical rehabilitation, with near continuous use during normal operations. The unit will be installed and used in a Spinal Cord Injury rehabilitation center. Unit must have the capability to simultaneously work the leg and arm/upper body at the same time engaging approximately 80% of the skeletal muscles. Unit must have the ability to separate exercise functions and provide either an upper or lower body workout independently with the ability to set passive movement independently as well. Equipment must be highly mobile with the ability to relocate easily without any additional setup or stabilization needed. Unit must provide spasm control technology that can monitor muscle condition throughout the therapy session and stop movement once a spasm is detected and restart movement in the opposite direction until the spasm has stopped. Training Modes of Operation Passive (motorized movement); motor-assisted (supported movement); With patient muscle power (against resistance); Intervals (alternating between passive and active modes of operation). Unit will have patient interaction and engagement tools such as games and active encouragement. Biofeedback Requirements Equipment must provide biofeedback during and after training and have the capability to provide results to include: Duration, distance, symmetry, total energy, tone, average and maximum force. The Contractor may be required to adjust the delivery date from the date specified in the contract award document. A government-requested delivery delay of up to 90 days after the delivery date specified in the contract award shall be at no additional cost to the government. Delivery Location: Contractor shall deliver all equipment to the Rocky Mountain Regional Veterans Affairs Medical Center (RMRVAMC), 1700 N. Wheeling Street, Aurora, CO 80045. Delivery Markings: Contractor shall deliver items in manufacturer's original sealed containers with manufacturer s name marked thereon. Deliveries shall be marked with the PO, contract number, and On-Site POCs detailed in 2.6.1.4. Delivery will be coordinated through the On-Site Point of Contact. SITE CONDITIONS There shall be no smoking, eating, or drinking inside the delivery area at any time. Delivery personnel must comply with all posted site safety requirements, to include the wearing of PPE if required. INSPECTION AND ACCEPTANCE 3.1. The Contractor shall conduct a joint inspection with the On-Site Point of Contact upon completion of delivery. 3.2. In the event deficiencies are identified, the Contractor shall provide date when the identified deficiencies will be addressed if not addressed on the date of delivery. 3.3. The Contractor shall conduct a joint inspection with the On-Site Point of Contact after addressing all deficiencies. 3.4. All deficiencies identified during joint inspections shall be fixed by the Contractor prior to government acceptance of the item. 3.5. Disputes shall be resolved by the Contracting Officer. Operation and Maintenance Manuals Binders - Quantity (2) each for items identified in section 2.5. 4.2. Deliver compilation of all manufacturer recommended maintenance schedule and operation materials packaged in binder(s) to On Site Point of Contact upon completion of delivery. 5. OPERATOR TRAINING 5.1. Contractor shall provide all training materials at time of delivery. 5.2. Training will include 6 hours, onsite training for FES for up to 8 physical therapists. 5.3. Training must be complete and equipment ready for use NLT 30 June 2019. 6. PROTECTION OF PROPERTY 6.1. Contractor shall protect all items from damage during delivery. The Contractor shall take precaution against damage to the building(s), grounds, and furnishings. The Contractor shall repair or replace any items related to building(s) or grounds damaged accidentally or on purpose due to actions by the Contractor. 6.2. The Contractor shall perform an inspection of the building(s) and grounds with the On-Site Point of Contact prior to commencing work. The Contractor shall be responsible for repairing or replacing any items, components, building(s), or grounds damaged due to negligence and/or actions taken by the Contractor or its employees. The source of all repairs beyond simple surface cleaning is the facility construction contractor (or appropriate subcontractor), so that building warranty is maintained. Concurrence from the VA Facilities Management POC and On-Site Point of Contact is required before the Contractor may perform any significant repair work. In all cases, repairs shall utilize materials of the same quality, size, texture, grade, and color to match adjacent existing work. 6.3. The Contractor shall be responsible for securing the items identified in section 2.5 and its own work tools and equipment during delivery. 7. SECURITY REQUIREMENTS 7.1. Security background checks are not required. 7.2 The C&A requirements do not apply, and a Security Accreditation Package is not required. 8. WARRANTY 8.1. The contractor shall provide all manufacturer(s) warranties on all parts and labor at time of delivery. 8.2. The warranties shall include all travel and shipping costs associated with any warranty repair. 8.3. A combined warranty for the two (2) each Letto2 devices with integrated FES unit for a total of 3 years. 8.4. A combined warranty for one (1) each Muvi device with integrated FES for a total of a total of 3 years. 8.5. A warranty for the one (1) each FES unit purchased for the existing FES unit for a total of 3 years.
