Description
THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR QUOTE. The NCO 12, Great Lakes Acquisition Center, 115 S. 84th Street, Suite 101, Milwaukee, WI   53214, is performing market research to determine if there is a sufficient number of qualified: (1) Service Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); or (3) Small or emerging small business firms, or (4) Other Than Small business firms, to provide Procedure Chairs, MTI brand name or equal, for the Clement J. Zablocki VAMC in Milwaukee, Wisconsin. General Requirements: BRAND NAME (MTI 550) OR EQUAL: Two (2) Procedure Chairs shall include at a minimum the following characteristics. - Power back, tilt, and foot controls. - Foot must have a minimum extension of 5 inches. - Ability to hold a minimum of 550 pounds. - Be ADA Compliant. - The upholstery shall be seamless and easy to remove for cleaning. - Include safety lockout system to help keep the chair in a safe position. - Power supply should be 110 - 120 Volts. - Shall come with a debris tray to catch biohazard material. - Debris tray should be a minimum width of 20 inches. - Length of the debris tray shall be a minimum of 20 inches. - Debris tray height shall be between 5 - 10 inches. - Chair shall be a minimum length of 70 inches at full extension. - Chair shall have minimum height of 15 inches. - Chair shall be a maximum width of 30 inches. - Color of the upholstery shall be Midnight Blue to match existing interior design. - Headrest shall be rectangular. - Chair shall include transfer support and bed rails. - Base of the chair shall be mobile. All interested firms who can meet the requirements stated above should respond, in writing, INCLUDING ALL THE FOLLOWING: Company name. Address. Point of contact (name/title/telephone number/e-mail address). FSS / SEWP Contract number, if requirement is on schedule. DUNS number. Tax ID number. Indication of which business category (SDVOSB, VOSB, Small Business or Other Than Small Business) for which the organization qualifies. A capability statement that addresses the organizations qualifications and ability to provide the requirement depicted above. Documentation from manufacturer (i.e. correspondence from manufacturer) of your firm being an authorized distributor for the manufacturer.
