Page 2 of 2 VETERANS AFFAIRS MEDICAL CENTER, BUTLER, PA GENERAL INFORMATION FOLLOWED BY A TENTATIVE STATEMENT OF WORK NOTICE: This is not a solicitation but rather a Request for Information (RFI) (Sources Sought) to determine capability of potential sources and is for information and planning purposes only. Veterans Health Affairs, Network Contracting Office 4, is issuing this request for information/sources sought notice in order to identify capable firms to perform inventory services for the Butler VA Medical Center. Responses must be submitted by 4pm EST. Time, June 6, 2019. Submit responses to the information requested above via email to jameel.gordon@va.gov All SDVOSB and VOSB firms that respond shall include proof of CVE certification via www.vip.vetbiz.gov. All small business firms that respond shall include proof of small business status via their Representations and Certifications in accordance with (FAR 4.1102 Policy). While SDVOSB/VOSB contractors are preferred, all capable contractors are welcome to respond to this sources sought notice for market research purposes, and shall include as part of their response a brief capability statement that covers the information in the following tentative statement of work The results of this market research will assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., socioeconomic set-aside, full and open competition, etc.). VA assumes no responsibility for any costs incurred associated with the preparation of responses. Tentative/Sample Statement of Work: Background: The Department of Veteran Affairs Medical Center located at 353 North Duffy Road Butler, Pa 16001 uses both in house and a contracted dental lab for various stages in the fabrication process of oral prosthesis. The contractor shall provide all labor, supervision and responsibility required to deliver dental lab services to include work completed by subcontractors for the items listed in this schedule. No work will be outsourced oversees. All dental appliances will be fabricated inside the United States using American Dental Association (ADA) approved materials. This contract will be for 5 (five) years and will include one base year, from date of award, plus 4 (four) option years that will be exercised at the government convenience. OBJECTIVES: An overview of this SOW will include goals and objectives. VA Butler healthcare has a need to continuously utilize dental lab services for Veterans requiring special dental lab work. I STATEMENT OF AWARD The government intends to award a contract as a result of this solicitation depending on the number of responsible, complete and responsive offers received. Services provided will be of high quality, efficient and cost effective. The attached Services Price Cost spreadsheet is an estimate of the quantity of services and may include more or none of the estimate quantity listed. The contractor will complete all cases within 14 (fourteen) calendar days of pick up. Pick up and delivery will be done on a weekly basis during normal business hours which are Monday through Friday 7:30 to 4:00 pm. II STANDARDS OF PERFORMANCE The contracted dental laboratory shall fabricate all prosthesis and perform all steps to the highest standard of quality with special attention to detail and adhering to the dental prescriptions. Contractor will supply VA dental clinic with prescription pads to include name address and telephone number of contractor. This solicitation/contract includes regulations under the Buy American Act; VA Butler Healthcare requires that all dental appliances supplied to the dental clinic are manufactured in the United States using high quality materials that meet ADA regulations. Dental laboratories must also comply with the regulations from the Food and Drug Administration (FDA) quality systems/good manufacturing practices for products distributed in us dental labs. Contractor will have all licenses, permits and certification required by law. Current copies must be provided to the CO/COR upon request. Each case will be identified by only the first and last name between the government and the contractor. No customer specific Personally Identifiable Information (PII) will be used at any time. Contractor will be required to complete VA Privacy and Information Security Awareness and Rules of Behavior in TMS (VA 10176) upon award of contract and yearly thereafter, copy of completion certificate will be forwarded to the COR. Contractor or contractor representative will be available by phone during normal VA hours. If it is found that returned prosthesis is not compliant with the prescription, the contractor will remake the item at no additional cost to the government. III PLACE OF PERFORMANCE The place of performance will be at the contractor s location. IV FABRICATION MATERIALS The awarded contractor will use High Noble Alloys to fabricate all crown and bridge prosthesis. High Noble Alloys are alloys that have a composition of at least 40% gold. All gold restorations will be of a Type III yellow/white gold alloy according to the prescriptions. The contractor will provide Alloy labels issued by the vendor to identify the content elements used for each individual case. Contractor will use porcelain with a Coefficient of Thermal Expansion (CTE) compatible with the alloy substructure. All other materials ( wax, acrylics, chrome cobalt) will be in compliance with ADA regulations. All artificial teeth will be supplied by the VA dental clinic. CROWN AND BRIDGE/ ALL PORCELAIN All procedures from pouring preliminary impression, custom tray, stent, final cast and trimming die. For all single and multiple unit porcelain fused to metal, full gold crown, survey crown, veneers and all porcelain crowns. Fixed partial denture work may include metal rest seats, porcelain butt margins, metal occlusal, metal and porcelain guide planes, precision attachments, ERA attachments, with guide planes(in metal and porcelain) or retro fitting crown to existing Removable Partial Denture as requested. Repairs (soldering, porcelain addition, restack) to existing crowns and or fixed partial frameworks. Dental implant supported crowns and fixed partial dentures will be fabricated to include custom abutments for cemented and screw retained implants. VA Butler will be billed for the desprued weight of all precious metal crowns. REMOVABLE ( The VA will supply all teeth) All procedures from pouring preliminary impression, custom trays, bite rims, metal frameworks, partial dentures, complete dentures, night guards, bleaching trays, precision connectors, flexible partials and implant support dentures. Repairs may require reline, rebase, repair, adding teeth, adding clasps, solder and laser welding for all acrylic, flexible and metal prosthesis. Complete and partial dentures may include, resets, implant retained, implant supported, ERA style connectors, one tooth to complete restorations with varying occlusal schemes and various shades of denture acrylic. V QUALITY OF WORK Communication back and forth between the VA Butler Healthcare and the vendor is important to maintain and coordinate the quality assurance aspects of this contract. The contractor will comply with the Dental lab contract performance measures listed in Attachment A, Contractor Performance Report. Failure to routinely meet the standards may be cause to pursue Termination for Default in accordance with federal acquisition regulations. Remakes: If it is found that any laboratory item is not compliant with the prescription, the contractor shall remake the item at no additional cost. The Contracting Officer Representative (COR) will notify the vendor in writing on a monthly basis of any issues concerning quality complaints of dental laboratory items providing detailed information for the vendor to correct identified VI DELIVERY SCHEDULE The contractor should/need to be available in person within one hour after receiving a request from VA Butler Healthcare Center for delivery and chairside services. Contractor shall consult with the appropriate dental provider at VA Butler Healthcare regarding any specific dental laboratory order. Pick-up and delivery will be made at the VA Butler Healthcare Dental Clinic 353 North Duffy Road Butler Pa 16001 at an agreed upon time coordinated with the CO ( Contracting Officer) /COR ( Contracting Officers Representative) and the selected contractor. Contractor will be required to sign in/out upon arriving at VA Butler Healthcare facility. Commercial carriers such as UPS, Fed EX, USPS are unacceptable substitutes for contractor provided couriers. Contractor will have a contingency plan in the event equipment or supplies are unavailable for scheduled dental work. Contractor will give 48 hour notice of any anticipated/unanticipated delay in the delivery schedule. The contractor will provide weekly pick up/delivery during normal business hour. Delivery and pick up will not take place the next business day following a holiday. Federal Holiday are New Year s Day, Martin Luther King Day, President s Day, Memorial Day, July 4th, Labor Day, Columbus Day, Veteran s Day, Thanksgiving and Christmas. When a holiday falls on a Sunday, the following Monday shall be observed as a legal holiday by the U.S. Government agencies. When a holiday falls on a Saturday, the preceding Friday shall be observed as a legal holiday. Also included would be any other day specifically declared by the President of the United States to be a National Holiday.
