Description

This Request for Quote (RFQ) is for a Nonpersonal Healthcare Service Contract for a Contractor to provide Optometrist to the Chinle Comprehensive Health Care Facility (CCHCF), Navajo Area Indian Health Service (IHS) throughout the duration of the contract period of performance, which is from June 8, 2019 through October 31, 2019. Services shall be performed in accordance with the attached Performance Work Statement (PWS) CSU-19-01 OD dated May 21, 2019. Refer to the PWS, Section 5.2 for Work Schedule. The assignment is through the stated period of performance or until the position is filled with a permanent hire. In such instances, the provider will be released from contract assignment. NOTE: THIS NOTICE WAS NOT POSTED TO FEDBIZOPPS ON THE DATE INDICATED IN THE NOTICE ITSELF (30-MAY-2019); HOWEVER, IT DID APPEAR IN THE FEDBIZOPPS FTP FEED ON THIS DATE.
