- This modifies the synopsis issued on 21 May 2019 as follows: Base pass forms and their instructions are revised and replaced with the following: (1) Contractor Memo and III Log - Memo must follow format, complete the items listed in red color font - For Contractor Memo, leave Tracking # field blank - Follow instructions as provided in How to fill out III Log (2) Contractor Affidavit - To be completed by each individual attending the site visit - Type/print information clearly and legibly - Must contain actual signature (no digital signatures will be accepted) (3) Base Access List (BAL) form - List all attendees in alphabetical order by last name - Provide contact information and signature of POC (4) Provide copy of U.S. passport/Green Card AND government issued photo ID (5) Disclosure and Release Form (6) Foreign National Access Request Form (as applicable) - To be completed by all foreign nationals attending the site visit and must be accompanied with a copy of work permit (Employment Authorization Document (EAD)) or document permitting stay/work in U.S. (I-94 Form), and copy of foreign passport - Not required for foreign nationals who possess a Green Card - List all attendees in alphabetical order by last name All other information in the synopsis issued on 21 May 2019 remains unchanged. NOTE: THIS NOTICE WAS NOT POSTED TO FEDBIZOPPS ON THE DATE INDICATED IN THE NOTICE ITSELF (30-MAY-2019); HOWEVER, IT DID APPEAR IN THE FEDBIZOPPS FTP FEED ON THIS DATE.
