(i) This is a combined synopsis/solicitation for NGENUITY 3D VISUALIZATION SYSTEM, BRAND NAME OR EQUAL, as prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. A firm-fixed price purchase order is anticipated. (ii) The solicitation number is 36C24419Q0736 and is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 (iv) This solicitation is set aside 100% for Service-Disabled Veteran-Owned Small Businesses and the associated NAICS code 339112 has a small business size standard of 1000. Brand: _______________________________________________________________________ Model No.:____________________________________________________________________ SON/Salient Characteristics NGENUITY w/Data Fusion 3D System Statement of Need Contract Title: NGENUITY w/Data Fusion 3D System Background: The Lebanon VA Medical Center (LVAMC) has a requirement for one (1) NGENUITY 3D Visualization System. This device is utilized for Ophthalmology related surgical procedures in the OR. It is used in conjunction with the Microscopes we have in the OR used for Ocular Surgery. Scope of Work: The NGENUITY unit is an add-on to the Microscope used in the OR to perform Ocular Surgery. It allows the surgeon to operate without looking through the microscope. Instead the surgeon wears glasses and looks at a large LCD television mounted on the wall or a stand close by. The 3D Visualization System consists of a 3D stereoscopic, high-definition digital video camera and workstation to provide magnified stereoscopic images of objects during micro-surgery of the eye. It acts as an adjunct to the surgical microscope during surgery displaying real-time images which benefits the teaching environment in the OR for this Residency Program in the Ophthalmology Discipline. Compared to traditional analog microscopes, Ngenuity delivers up to 19 percent increased magnification to amplify the view for intricate tasks, up to 2.7 times extended depth of field to maintain focus across an expanded surgical space, and up to 19 percent increased depth resolution to resolve fine details when managing challenging pathologies.1 Salient Characteristics of the ALCON Heads Up Unit: 3D HD Digital Video Camera w/workstation 3D High Dynamic Range Camera 3D 4K OLED Monitor Digital Filtration enabling customized views which can highlight ocular structures and tissue layers. Facilitates operating in a lower light level which fosters a positive surgical experience. Additional Information: Many ophthalmologists develop neck pain early in their career, and some require surgery due to the substantial amount of time spent looking down into the microscope during surgery. This new technology eliminates the ergonomic issues of operating via the microscope. This equipment will be used on more than 50% of all cataract surgeries within the 1st year of installation. There are approximately 500 cataract surgeries performed at Lebanon VAMC per year. Use would increase in subsequent years to approximately 75%. Delivery: Delivery shall be within thirty (30) days from the time of award. The NGENUITY and accessories shall be delivered to the Lebanon VAMC located at 1700 S. Lincoln Ave., Lebanon, PA 17042; Attn; Warehouse. Bio-Medical Engineering will need to perform a safety check and enter the equipment in to the system for annual Preventive Maintenance (PM) checks. Placement in the LVAMC OR shall be coordinated through the OR Nurse Manager, and/or the Contracting Officer Representative. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and the following clauses AND instructions are added as addenda: CLAUSES: 52.211-6 Brand Name or Equal (AUG 1999) 52.214-21 Descriptive Literature (APR 2002) 852.211-73 Brand Name or Equal. (JAN 2008) 852-219-10 VA Notice of Total Service-Disabled Veteran-Owned Small Business Set-Aside (a) Gray market items are Original Equipment Manufacturer s (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers.   No remanufactures or gray market items will be acceptable.   (b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM.   All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. (c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract.   Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to  replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.   (ix) Evaluation of this requirement will be based on LOWEST PRICE TECNICALLY ACCEPTABLE (LPTA). (x) Offerors are advised to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer if has not been completed on SAM.gov.
