This notice is intended strictly for market research. The purpose of this Sources Sought notice is to determine interest and capability of businesses including (but not limited to) 8(a), HUBZone, Service Disabled Veteran-Owned Small Business, and Veteran-Owned Small Business firms relative to the North American Industry Classification Code (NAICS) 334510. The Department of Veterans Affairs is seeking both manufacturers and/or distributors of the exact match or functionally equivalent equipment/ services to the following listed above on Friday June 7 2019 No Later Than 4PM CST. STATEMENT OF WORK 1. Contractor shall provide new OEM Acquisition module for Phillips EPIQ 5. Refurbished parts/ components are not acceptable. Model: Phillips EPIQ 5 Serial Number: US516C0823 Location: 4800 Memorial Drive Bldg 24 (Attn: BIOMED) Waco, TX 76711 2. Requirements a. Contractor shall deliver parts to equipment location and provide return shipping label if exchange is required. b. Contractor shall only provide new OEM parts to complete the repair. Refurbished and/ or third party replacement parts are not to be used for this repair. d. Any part and/ or service not covered by this service contract will not be installed without first procuring a formal purchase order from the VISN 17 Contracting Officer assigned to this 2237. e. Contractor shall provide written documentation certifying that all safety tests have been performed, and that the unit is safe for patient use. 3. References a. Not applicable 4. Delivery a. Contractor shall perform all requirements to complete this contract within 30 days after being awarded. 5. Quality assurance surveillance program (QASP) a. Contractor shall provide written proof that all parts/ materials used in the performance of this contract were new OEM parts/ materials. 6. Security Consideration a. Contractor shall be escorted at all times during the performance of this contract. b. Contractor shall not access/ utilize VA computer systems in the performance of this contract. c. Contractor shall not access any information protected under the Privacy Act of 1972 or HIPPA. d. Contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract. 1. The C&A requirements do not apply, and a Security Accreditation Package is not required. 2. This service is a repair of the above unit, no sensitive information should be accessed, to ensure this; a Biomedical Technician will escort the contractor the entire time they are on-site and have all studies currently stored on the unit deleted before contractor arrives.
