Description
This is a Pre-Solicitation Notice for a commercial requirement for VISN 6 Eyeglasses for Veteran beneficiaries in Virginia and North Carolina. The Department of Veterans Affairs (VA) has an ongoing program to provide eyeglasses to Veteran beneficiaries in seven (7) medical centers (Asheville VAMC, Durham VAMC, Fayetteville HCC, Hampton VAMC, Richmond VAMC, Salem VAMC and Salisbury). In addition, services will also be needed at Community Based Outpatient Clinics (CBOC) and Health Care Centers (HCC) that provide optical services. The VA currently operates CBOCs or HCCs; in Charlotte, NC, Kernersville, NC, Hickory, NC, Greenville, NC, Wilmington NC and Morehead City. The contractor must possess all licenses, permits, accreditations and certificates as required by law. The contractor must perform the required work in accordance with the American National Standard Requirements for Ophthalmic Lenses and Frames and the Food and Drug Administration (FDA). The contractor shall meet all Federal, State, and local fire and life safety codes. The contractor must participate in the VA Credentialing and Privileging process prior to the activation of the contract. Once granted privileges, the contractor must adhere to the VA Bylaws and Rules, as well as to the operational policies and procedures of the VA Medical Center. Emphasis is placed in the provision of timely, high quality services and interacting appropriately, professionally and productively with VA patients and staff members at all levels of organization. In addition, the contractor must interact with the designated COR as directed by the CO. The contractor shall maintain an adequate inventory of all items covered by this requirement and shall provide a similar or equal item if stock is not immediately available or in case of emergency. Any proposed substitutes for, or changes to; the approved frames must be approved, in writing, by the COR. In addition, new frame samples shall be submitted prior to each contract renewal and must be approved by the COR s prior to contract award. The contractor shall be staffed and stocked with adequate inventory to render prompt service and shall be equipped and trained to render satisfactory and courteous service always to Veteran beneficiaries, VA staff and public. The contractor will have a minimum of two staff employees available always during normal business. The contractor shall have the capability to receive orders via facsimile (fax) transmission located in a secure location not open to the public or through encrypted email capability. The fax machine shall be independent from the contractor s established phone line and be able and willing to accept faxed orders. The contractor shall maintain a log of all work transmitted via fax, computer, and telephone. The contractor will utilize existing space within each VAMC, CBOC or HCC to provide services to eligible Veterans entitled to government provided eyeglasses and retail sales to the public (VA employees, volunteers and visitors). These services shall typically be required Monday Friday, 8:00 a.m. 5:00 p.m. except for Federal holidays. The NAICS Code for this requirement is 339115 Ophthalmic Goods Manufacturer with a size standard of 1,000 people. This acquisition is set-aside for 100% Service Disabled Veteran-Owned Small Businesses. A Firm Fixed-Price Requirements type contract with a one (1) year base period and up to four (4) one (1) year option periods, to be exercised at the Government's discretion is contemplated under the solicitation. A formal Request for Proposal (RFP) in accordance with Federal Acquisition Regulation (FAR) Part 12 Acquisition of Commercial Items and FAR Part 15 Contracting by Negotiation is included with this notice. Potential sources are hereby notified that a solicitation will be issued for these services following this Pre-solicitation Notice. Do not respond to this Pre-Solicitation Notice.
