THIS IS A SOURCES SOUGHT NOTICE ONLY. This is not a solicitation for bids, proposals, proposal abstracts, or quotations. The purpose of this Sources Sought Notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. The responses received from interested contractors will assist the Government in determining the appropriate acquisition method. The Department of Veterans Affairs (VA), NCO 21- is requesting procurement information for Brand Name or Equal commodities for a ceiling lift, for a firm fixed-price contract for the San Francisco VA Medical Center at 4150 Clement St., San Francisco, CA 94121 CLIN 0001 Description: Ceiling Lift (Brand Name or Equal: Arjo Maxi Sky with KWIKtrack system) QTY: 9 ea. CLIN 0002 Description: Hardware Kit QTY: 1 ea. CLIN 003 Description: Fees and Installation QTY: 1 ea. Salient Characteristics: Essential/significant physical, functional, or performance characteristics Ceiling lift must have at least 600 pounds weight capacity Ceiling lifts systems and equipment must be delivered on or after November 14, 2019. Ceiling lift motor should not weigh more than 28 pounds Ceiling lift must have a scale Ceiling lift must have LED indicator for maintenance required. Ceiling lift must have electronic microprocessor soft-start and stop motor control Ceiling lift must have power on indicator and an emergency brake system in case of mechanical failure Ceiling lift must have electrical up and down emergency buttons. Ceiling lift must have emergency stopping device (pull cord) accessible from the ground. Ceiling lift must have a low battery indicator (audible and visual LED) Ceiling lift must have charging indicators; blinking yellow-charging, full yellow-charging complete Ceiling lift must have a lifting speed 1.2 /sec at 600 pounds Ceiling lift must have a hand control San Francisco VA Medical Center 4150 Clement Street San Francisco, CA 94121 Statement of Work: General Specifications for purchase of Ceiling lifts 1. Background The critical needs for ceiling lifts system: is to prevent patient and staff injuries at the San Francisco VA Medical Center. These ceiling lifts systems will allow patient care to be delivered in a safe manner, for example during bathing, rehabilitation, ambulating, transfer from bed to chair. This is critical to provide excellent care for the patient. This will provide good outcome for the patient such as going home, going back to work, and being discharge from the hospital sooner. 2. Project Scope The San Francisco VA Medical Center is looking to add nine (9) ceiling lifts to: occupational therapy BA-118-208, inpatient rooms 2a61-203, 2a-67-203, CLC-shower room 1C-115-208, Women s Clinic 1B50A-203, GA172 module 3-200, GA152 Module 1-200, Medical Practice 1A212-200 (exam room for wound care), and in patient room 3B50-203. Purchase will also include the rails and other equipment needed to install and secure the ceiling lifts to the ceiling/or walls. 3. Specifications Essential/significant physical, functional, or performance characteristics for the ceiling lifts must have 600 pounds weight capacity, ceiling lift motor should not weigh more than 28 pounds, have a weighing scale, must have electronic microprocessor soft-start and stop motor control, must have power on indicator and an emergency brake system in case of mechanical failure, must have electrical up and down emergency buttons, must have emergency stopping device(pull cord)accessible from the ground, must have a low battery indicators; blinking yellow-charging, full yellow-charging complete, must have a lifting speed 1.2 /sec at 600 pounds, and must have a hand control. Ceiling lifts shall meet all underwriters laboratory requirements, life safety codes, national electric codes, fire safety codes, and all other applicable federal state and local codes. Appropriate documentation will be provided. Ceiling lifts shall comply with ISO 10535 (Hoists for the transfer of disabled persons-requirements and test methods. Ceiling lift manufacturer must allow and warrant the use of their ceiling lifts and tracks. The lifts shall be mounted (using ceiling or floor support as determined by building structure and room covering layout needs) to lifting capability for the maximum area possible within each room. Lifts must be able to pick patients off the floor. 4. Training Must provide 24 hour training of staff within a two month period on the use of ceiling lifts. 5. Delivery Location: San Francisco VAMC 4150 Clement Street San Francisco, CA 94121 6. Delivery Requirements: The Vendor will not deliver the equipment until after November 1st, 2019. The Vendor shall notify the COR to plan the delivery date. This notification shall plan and coordinate with the COR, Project Engineering, occupational therapy, nursing managers, logistics, and occupational therapist supervisor. RESPONSES MUST INCLUDE THE FOLLOWING INFORMATION: Company Name, Address, Size Standard, DUNS #, Cage Code Point of Contact name, phone number, and email address NAICS Code 339112 and Small Business Size Standard 1000 employees, MUST be assigned to company Capability Statement *NOTE Acquisitions will only be set aside for SDVOSBs/VOSBs if two or more responses are received with the required information listed above, by the due date specified below. Responses are due by 4:00 PM (EST) on Friday, 06/14/2019. No submissions will be accepted after this date and time. This is a Sources Sought Notice and submissions will be used for informational and planning purposes only. GREY MARKET ITEMS: The Vendor shall be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor, or authorized reseller for the proposed equipment/system such that the OEM warranty and service are provided and maintained by the OEM. The vendor shall provide new items ONLY; no remanufactured or grey market items. All items must be covered by the manufacturer s warranty. The offeror shall provide evidence of its authority to distribute the proposed equipment/system at the time of submission of its quote/offer. Please note that such evidence is subject to verification by the Contracting Officer at his/her discretion. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. The Government will treat any information received as proprietary and will not share such information with other companies. Any organization responding to this Sources Sought Notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. The Government may or may not issue a solicitation as a result of this announcement. There is no solicitation available at this time.
