THIS IS A REQUEST FOR INFORMATION NOTICE ONLY. This notice does NOT constitute a request for proposal, request for quote, or invitation for bid. No formal solicitation document exists at this time. We are seeking information for market research purposes only. The Government will not pay for any information solicited. The VHA Network Contracting Office 5 (NC0 5) is conducting a market survey to find qualified vendors who can provide the following service for the VA Hospital in Baltimore Maryland. DESCRIPTION The Baltimore Veteran Affairs Medical Center (VAMC) in Baltimore Maryland, requires Maintenance on a Wizard 2 multi detector gamma counter 9/1/2019 8/31/2020 plus three (3) one (1) year options. The contractor shall furnish all labor, materials, tools, equipment and supervision necessary to provide collection, storage, removal, transportation, and disposal of non-hazardous solid waste. This is a total set aside for Veteran Owned Small Business. NAICS code is 811219 REQUIREMENTS Vendor shall have proof of experience maintaining a Wizard 2 multi-detector gamma counter. The NAICS 811219 is applicable to this acquisition; Interested firms are reminded that in accordance with FAR 4.1201(a) Prospective contractors shall complete electronic annual representations and certifications at the System for Award Management (SAM) as a part of required registration in conjunction with FAR 4.1102(a) that states, Prospective contractors shall be registered in the (SAM) database prior to award of a contract or agreement. Therefore, due to time constraints, vendors must be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award. Your response is required by 1:00 PM EST on Thursday June 13 2019. STATEMENT OF WORK (SOW) 1. Contract Title: Geriatric Research, Education and Clinical Center (GRECC) 10 North Greene Street, Baltimore Md 2. Background: GRECC has funded researchers that utilize a Wizard 2 multi-detector gamma counter as part of their research projects which include having certain labs performed. The counters require yearly maintenance to ensure counting efficiency and accuracy. This service will be required yearly and need option years for the ongoing research being funded and provided. 3. Scope: As the investigators /lab technicians are performing research experiments, they may require that certain labs be provided for their research. The Contractor shall make sure the equipment is maintained to ensure accuracy and efficiency. 4. Specific Tasks: The Contractor shall provide the required maintenance service to the equipment to ensure accuracy in the lab results according to the investigator s protocol. 4.1 Task 1 - Enterprise Management Controls: 4.1.1 Subtask 1 - Integration Management Control Planning: The GRECC Laboratory Supervisor will ensure that the equipment is maintained accordingly. 5.Deliverables: Contract Management Plan COR will perform monthly contacts with the Investigators for the performance and usage and follow up. 6. Performance Monitoring: The GRECC Laboratory Manager will oversee the maintenance of the equipment and will address any issues directly with the vendor. The Lab Manager will discuss with the COR any performance issues relative to this contract. 7. Security Requirements: The Contractor shall be escorted at all times in the VAMHCS lab area. 8. Inspection and Acceptance Criteria: GRECC will provide the necessary back up documentation in support of charges on a monthly basis. 9. Risk Control: N/A 10. Place of Performance: The equipment is located in the GRECC laboratory on 3D. 11. Period of Performance: The period of contract will be from September 1, 2019 thru August 31, 2020 with 3 option years, making the entire period of performance 48 months. 12.
