- The Department of Veteran Affairs, Dorn VA Medical Center, 6439 Garners Ferry Road, Columbia, SC 29209 has a requirement for CABLE TV and WI-FI for the lobby area and the POD areas (Patient Exam Rooms) for the Greenville Outpatient Clinic (CBOC). The Government expects to award to a single contractor a contract which shall be Firm Fixed Price with a firm quantity and no option years. This requirement is being set-aside for Service Disabled Veteran Owned Small Business participation only. NAICS code for this requirement is 515210 with a small business size of 500 employees. Only offers from those firms regularly established in the business of providing this type of equipment will be considered. Subcontracting will not be allowed. All responsible sources may submit a proposal, which shall be considered. STATEMENT OF WORK Service-Cable TV Services Greenville Multi-Specialty Community Based Outpatient Clinic (MS-CBOC) Greenville, South Carolina GENERAL: The contractor shall furnish all labor, materials, tools, equipment, travel, supplies, and incidentals necessary to provide and supply Cable TV Services at the Greenville Multi-Specialty Community Based Outpatient Clinic (MS-CBOC), 41 Park Creek Drive, Greenville, SC 29605. BACKGROUND: The Columbia, SC Veterans Affairs Health Care System (HCS) currently includes five Community-Based Outpatient Clinics (CBOCs), as well as two MS-CBOCs that are geographically dispersed throughout the State of South Carolina in order to provide health care services for our Veteran population. The Greenville MS-CBOC is an approximately 78,000 GSF building that provides health care services to the Greenville area Veterans. These services include Primary Care, Mental Health, Laboratory, Tele-Health, Physical Therapy, Occupational Therapy, Audiology, Optometry, Dental, Pharmacy, Radiology, and other services for the Veterans. PERIOD OF PERFORMANCE: Base Year: TBD 1st Option Period TBD 2nd Option Period TBD 3rd Option Period TBD 4th Option Period TBD PLACE OF PERFORMANCE: Department of Veterans Affairs Columbia, SC VA Health Care System (HCS) Greenville Multi-Specialty Community Based Outpatient Clinic (MS-CBOC) 41 Park Creek Drive Greenville, SC 29605 SCOPE: The contractor shall furnish all labor, materials, tools, equipment, travel, supplies, and incidentals necessary to supply and deliver cable tv services within the Greenville MS-CBOC. The following areas: 12 tv s located within the main lobby, 5 tv s located within breakrooms, 1 tv located within the administrative conference room, 1 tv located within the administrative suite, 1 tv located within the canteen area. INVOICES: Payment will be made in arrears upon receipt of a properly prepared detailed invoice by the Contractor and submitted to the VA Financial Services Center (FCS): VA Financial Services Center P.O. BOX 149971 AUSTIN, TX 78714 A properly prepared invoice will contain: Invoice Number and Date Contractor s Name and Address Accurate Purchase Order Number Supply or Service provided Service period and location Total amount due CONTRACT ADMINISTRATION DATA: The Contracting Officer is the only person authorized to approve changes or modify any of the requirements under this contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitments or issue changes which will affect price, quantity, or quality of performance of this contract. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change will be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in costs incurred as a result thereof. DESIGNATION OF CONTRACTING OFFICER S REPRESENTATIVE: A VA Medical Center representative of the Contracting Officer shall be designated to represent the Contracting Officer in furnishing technical guidance and advice regarding the work being performed under this task order. The foregoing is to be construed as authorization to interpret or furnish advice or information to the Contractor relative to the financial or legal aspects of the task order. Enforcement of these segments is vested and is the sole responsibility of the Columbia VA HCS Contracting Officer.
