- The Air Force District of Washington (AFDW) Contracting Activity (PK) on behalf of the AFDW/Safety Directorate (AFDW/SE) is issuing this request for information as means of notifying the public of an upcoming requirement to support the requirement for LINE OPERATION SAFETY AUDIT (LOSA). AFDW/SE requires contractor support for threat and error identification during flight crew performance and flight operations. As outlined in the Federal Aviation Administration Advisory Circular Number 120-90 dated 27 April 2006, this process, commercially referred to as a Line Operations Safety Audit (LOSA), and provides a rigorous and methodical assessment of the safety strengths and weaknesses in normal flight operations of US Air Force UH-IN Iroquois aircraft and aircrews. A LOSA is a formal process that requires highly trained experienced observers to ride the jump seat during regularly scheduled flights to collect safety-related data on environmental conditions, operational complexity, and flight crew performance. A LOSA provides a diagnostic snapshot of strengths and weaknesses that AFDW/SE can use to bolster the "health" of its safety margins and prevent degradation. Confidential data collection and non-jeopardy assurance for crewmembers are fundamental to the LOSA process.
