Description
This Request for Information (RFI) is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI will assist the government in determining how to solicit; the information received from this is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. The Government is not obligated, nor shall it pay for any information received from potential sources resulting from this Sources Sought Notice. The Orlando VA Health System is seeking information on business concerns capable of providing Special Mode Transportation (SMT) for the Corporate Orlando VA Medical Center addresses are listed below: Orlando VA Medical Center (Lake Nona Campus) 13800 Veterans Way Orlando, FL 32827 Lake Baldwin Outpatient Clinic 5201 Raymond Street Orlando, FL 32803 William V. Chappell Jr. Outpatient Clinic (Daytona OPC) 551 National Healthcare Drive Daytona Beach, FL 32114 Viera Outpatient Clinic (Viera OPC) 2900 Veterans Way Viera, FL 32940 Clermont 805 Oakley Seaver Drive Clermont, FL 34711 Crossroads Annex 925 South Semoran Blvd Suite 114 Winter Park, FL 32792 Kissimmee Community Based Outpatient Clinic 2285 North Central Avenue Kissimmee, FL 34741 Tavares Community Based Outpatient Clinic 1390 E. Burleigh Blvd Tavares, FL 32778 Deltona Community Based Outpatient Clinic 1200 Deltona Blvd Suite 41-47 Deltona FL, 32725 The Contractor shall provide all transport vehicles, personnel, management, supplies, transportation, fuel, equipment, reports, and anything else necessary to successfully furnish Non-Emergency SMT service to the Orlando VA Medical Center (locations listed above). The contract term shall cover a Base Period, plus four (4) additional option years; the one (1) year option periods are subject to be exercised at the Government s discretion. The contractor responsibility shall cover non-emergency special mode transportation services within the OVAMC catchment area which consists of six counties: Orange, Seminole, Osceola, Brevard, Volusia and Lake County 365-days per year, 24-hours per day, 7-days per week. SMT transportation may be required to be provided to beneficiaries that live in the adjacent counties that border the six counties under the OVAMC catchment area. The successful vendor(s) will be required to continuously be compliant with all requirements, and meet or exceed the standards of, but not limited to, the Joint Commission and Florida State Department of Health Regulations. Vehicles shall meet all current applicable Federal, State and local specifications and regulations including, but not limited to, licensing, registration, and safety standards. Appropriate licensing by the State of Florida is required. All Contractor personnel performing contract services shall meet all qualifications and licensures required by Federal, State, County and Local Government entities from the place in which they operate. Drivers shall have a valid operator's license in accordance with Federal, State, County and Local Government requirements for the subject counties for the services they perform; and shall have successfully completed Standard First Aid Courses. Firms with interest and capability to include: Service-Disabled Veteran-Owned Small Business, Veteran-Owned Small Business, HUBZone Small Business, Small Disadvantaged Business, Women-Owned Small Business, Small Business and Large Businesses.
