Description
Sheridan VA Medical Center Sheridan, WY 82801 Fire and Smoke Damper Inspection Brief Description: 1.1 This statement of work is for performance of services for identification, mapping, testing and inspecting smoke and fire dampers in support of the Sheridan VA Medical Center. The contractor shall provide appropriate personnel for these duties at the Sheridan VA Medical Center, 1898 Fort Road, Sheridan, WY. a. Hours of work are defined as Monday through Friday from 7:30 a.m. to 4:00 p.m., excluding Federal holidays or as otherwise arranged with the Contracting Officer Representative (COR). b. The ten holidays observed by the Federal Government are New Year s Day, Martin Luther King Day, President s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran s Day, Thanksgiving Day, and Christmas Day, and any other day declared by the President of the United States to be a national holiday. 1.2 Background: The Sheridan VA Healthcare System (SVAHCS) has 14 buildings with dampers which are required to be tested. Six of the 14 were last tested in Dec 2012 and are now past due for inspection. One of the buildings (B87) was tested in 2018 on a contract that had funding expire before other buildings could be tested, thus the need for a new contract. The 6 buildings (4,8,9,71,71N & 86) which have been previously inspected and that will be included in this contract have approximately 373 dampers total. The remaining buildings (2,3,5,6,7,24 & 64) to the best of our knowledge and records have never been tested. Therefore, damper locations and numbers for those buildings are unknown. Building 87 was completed in 2018 and will not be required for testing until 2024 and will not be included in this contract. 1.3 Objective: The objective of this service contract will be to provide location, mapping and testing of all dampers on the facility in below identified buildings. SECTION 2 REQUESTED SERVICES: 2.1 Contractor shall provide services, equipment, staff, and tools necessary to locate, map, test and inspect all smoke and fire dampers in the below identified buildings at the Sheridan VA Medical Center, 1898 Fort Road, Sheridan, WY 82801. All services shall be in accordance with The Joint Commission (TJC), NFPA 80, 90A, 101 & 105, and any other applicable NFPA standards. Contractor will include documentation of all tested devices and must identify as a minimum which includes date inspected, unique device identification, device location, mapped location, status (pass or fail), the type and quantity of devices tested in each building, and a consolidated deficiency finding list per building. This is a base year contract with no option years as inspections are conducted every 6 years. Buildings included are: 2 (UNK),3 (UNK),4 (60),5 (UNK),6 (UNK),7 (UNK),8 (48),9 (13),24 (UNK), 64 (15),71 (26),71N (53),86 (179) 2.2 Locate, actuate and map dampers located in each of the identified buildings 2.3 Remove and reset fusible links on Dampers, to verify dampers full closure and if necessary replace any fusible link that is compromised (Per NFPA code). 2.4 Clear each damper of any debris that would impede damper operation. 2.5 Coordinate with safety Office and local fire department if needed to place the fire alarm system in test mode for any testing of devices. 2.6 Manually activate actuators on electric and pneumatic smoke and/or combination dampers to verify proper operation of the damper. 2.7 If there s no existing access to a damper, then proper size access doors, will be installed to facilitate the inspection. 2.8 Identify mapped locations on Life Safety Drawings to indicate the location of each damper, along with placement of identification stickers on ceilings or other locations to aid in locating dampers for future inspections. SECTION 3 - SPECIAL CONDITIONS: 3.1 Any assigned Contractor personnel must be able to pass and maintain a background clearance investigation for obtaining a Government-issued identification badge. 3.2 Provide and maintain documentation, reports, and records of inspections and testing in hard copy and electronic format in order to better assist the customer with record keeping (DOCX, XLS). 3.3 Provide qualified, certified & experienced technicians for all inspections & testing. 3.4Coordinate inspections and testing with the appropriate VAMC Personnel. 3.5 Ensure to notify the all facility personnel where testing is to be performed. 3.6 Perform the testing during normal hours (7:30am to 4pm, Monday thru Friday). Hours may be adjusted with approval of Safety Office prior to scheduling. 3.7 Follow-up on completion of any work subcontracted to ensure work is done as required in a timely manner. 3.8 Provide guidance and knowledge to aid in Sheridan VAMC meeting requirements as set forth by NFPA and TJC during the term of this contract. 3.9 Ensure only authorized personnel are performing work associated with this contract. This applies to any work sub-contracted as a result of this award. SECTION 4 - GOVERNMENT FURNISHED DATA: 4.1 Building plans and Operation and Maintenance manuals will be available for review. SECTION 5 - RESPONSIBILITIES 5.1 Government Authorized Individuals Contracting Officer's Representatives (CORs) will be appointed for individual projects under this Agreement. The COR will be responsible for technical monitoring of the contractor's performance and deliveries. The COR and the Contractor's Representative shall work together to ensure that all contractual requirements are being met. The COR will interpret specifications or technical portions of the work. The COR is not authorized to perform, formally or informally, any of the following actions: (l) Promise, award, agree to award, or execute any contract, contract modification, or notice of intent that changes or may change this contract; (2) Waive or agree to modification of the delivery schedule; (3) Make any final decision on any contract matter subject to the Disputes Clause; (4) Terminate, for any reason, the contractor's right to proceed; (5) Obligate in any way, the payment of money by the Government. Only a warranted CO is authorized to obligate funds on this or any other contract action. The contractor shall immediately notify the CO in writing if the COR has taken an action (or fails to take action) or issues directions (written or oral) that the contractor considers to exceed the above limitations. The contractor shall provide the CO information copies of all correspondence to the COR. SECTION 6 - CONFIDENTIALITY AND NONDISCLOSURE 6.1 Standard mandatory language for all task orders. SECTION 7 - CONTRACTOR PERSONNEL SECURITY REQUIREMENTS 7.1 Contractors will be required to obtain a contractor badge and wear it while performing work on VAMC property. 7.2 Notify VA Safety Office if access is needed to a controlled area requiring escort. SECTION 8 - CONTRACTOR REGISTRATION: 8.1 The contractor must have current registration through System for Award Management (SAM), https://www.uscontractorregistration.com/ SECTION 9 PRICING BREAK DOWN 9.1 Cost of inspection per damper tested.
