Page 3 of 3 IMPORTANT INFORMATION: This is a Sources Sought announcement for market research purposes only. The information gained will be used to determine potential qualified contractors, preliminary planning, possibly set-side, and the method of acquisition. THIS IS NOT A SOLICITATION ANNOUNCEMENT FOR PROPOSALS, QUOTATIONS OR BIDS. Therefore, no contract will be awarded from this announcement. No reimbursement will be made for any costs associated with providing information in response to this announcement or any follow-up information requests. This notice is not to be construed as a commitment by the Government for any purpose other than market research. REQUIREMENT: This requirement is for OSHA Certified Forklift Operator and Safety Training for the VA Supply Chain Workforce. Specifically, the Contractor will: Provide a training facility with classroom and hands-on forklift operation and safety training to accommodate up to 15 students during a single training event, multiple times per year. Training shall be held at contractor provided space within 10 miles from one of the below airports for east and west coast events. West Coast (indicate one airport) California (LAX, San Francisco International, or San Diego International) Arizona (Phoenix Sky Harbor PHX) Texas (Dallas/ Fort Worth International) Nevada (McCarran) Colorado (Denver International) Washington (Seattle/ Tacoma International) East Coast (indicate one airport) Maryland/ Virginia/DC (BWI, Dulles, or Regan) Provide hands-on forklift operator training on gas, diesel, or electric powered forklifts. Training equipment shall include Class I, II, III, & IV powered forklifts. Contractor is to provide the forklift(s) and any other applicable training materials. Provide a minimum of 16 training hours (preferably 8 hours of classroom training and 8 hours of hand-on training. Provide course material that meets the following objectives: Determine the risk(s) involved in operating the forklift truck Become familiar with OSHA standards Become familiar with the operations and functions of different types of trucks Prepare for visual check-up of the machine prior to work Obtain forklift license Pass the formal training test Complete and pass the forklift driving evaluation CAPABILITY STATEMENTS: Interested parties, small or large organizations, should submit capability statements for this request via email to Gary Ruc at gary.ruc@va.gov and Micah Kauzlarich at micah.kauzlarich@va.gov by Wednesday, 26 June 2019, at 11am Eastern Time. All respondents must be registered in the System for Award Management (https://sam.gov/SAM/) to receive federal awards. All veteran owned companies must be verified through the Vendor Information Pages (https://www.vip.vetbiz.va.gov) to be recognized as a veteran owned company by the VA. Information to be provided SHALL include, but is not limited to the following: Contractors information: Provide your company contact information, such as, name, address, point of contact, phone number and e-mail address; Type of Business: Small, Woman, Owned, Veteran Owned, etc.; Experience: Provide the number of years in business and a narrative description of prior experience performing similar contracts in terms or size, scope, and complexity (does not have to be a prior federal contract). Price: Provide your assumed price per student considering the requirements. For the Contracting Officer to decide that a set-aside can perused, pricing information must be provided. Without this, the Contracting Office cannot determine that pricing can be received at a fair and reasonable price for socio economic concerns. Existing Contracting Vehicles: If the respondent has a GSA FSS contract or another applicable contract, indicate so.
