Description
THIS IS NOT A SOLICITATION ANNOUNCEMENT The Department of Veterans Affairs (VA) has an ongoing program to furnish Artificial Limbs and related services to those veterans that require them and are eligible for assistance. VISN 8 Network Contracting Office (NCO) is conducting market research through this Sources Sought notice for businesses to provide Artificial Limbs and related services within a VA Medical Centers (VAMC) Bay Pines, Puerto Rico, Miami, North Florida/South Georgia, Orlando, Tampa, and West Palm Beach VA Healthcare System jurisdiction under the authority of Title 38 U.S.C. Section 8123. It is the intent of the Government to award a firm fixed price, blanket purchase agreement. The Government reserves the right to award multiple contracts. This contract will be for a period of one (1) year from date of award plus one (1) additional one year option. This sources sought announcement is NOT a request for proposals and the Government is NOT committed to award a contract pursuant to this announcement. The capability statements received from this market research are for planning purposes only and will assist the Government in planning its acquisition strategy. This is strictly market research and the Government will not entertain questions concerning this market research. All interested Offerors will need to respond to the future solicitation announcement. The NAICS code for this requirement is 339113. Responses to this notice should include the following: Company Name and Address Business size (large or small) DUNS number Socioeconomic status (Service-Disabled/Veteran-Owned Small Business (SDVOSB/VOSB), Hub Zone, 8(a), Small, Small Disadvantaged, Woman-Owned Small Business, FSS/GSA contract schedule holders or Large Business) Any pertinent information which demonstrates firm s ability to meet the above requirement. Vendors should also specify what areas of VISN 8 VA Healthcare they can provide services to: Bay Pines Puerto Rico Miami North Florida/South Georgia Orlando Tampa West Palm Beach If a solicitation is issued it will be announced at a later date and all interested parties MUST respond to that solicitation announcement separately form the responses to this announcement. Vendor Requirements: The contractor shall fabricate all artificial limbs in strict conformance to the prosthetic prescription that has been provided by each VISN 8 Prosthetic Clinic. The Contractor s facilities must be approved by the Facility Accreditation Program of the American Board of Certification (BOC). The contractor shall have a full-time American Board Certified (ABC) of Board for Orthotist-Prosthetist (BOC), or VA Qualified (VAQP), Prosthetist on staff at each of their locations in good standing. New technologies require proof of training and only those prosthetists specifically trained or completing training for purposes of providing an item by the time of delivery of the item may provide the item and follow up services to a veteran. The fitting and alignment of the following prostheses/services must be performed under the supervision of a Prosthetist who has met the applicable education, training, and licensing requirements: Ischial Containment Prosthetic Designs Microprocessor knee Silicone suction sockets (3S) Tec Harmony System The training of the Prosthetist shall include Computer Aided Design/Computer Aided Manufacture (CAD/CAM), Longs Technique, National Student Nurses Association (NSNA), and/or educational seminars specific to the fabrication and servicing of these components that were presented by organizations such as National Association for the Advancement of Orthotics and Prosthetics (NAAOP), American Academy of Orthotics and Prosthetics (AAOP), American Orthotic and Prosthetic Association (AOPA), and International Association of Orthotics and Prosthetics (IAOP). VA shall require the Contractor to deliver items ordered under this contract no later than 30 calendar days after receipt of a purchase/delivery order. Quotes must be returned to the requestor within 10 days of receipt of request for quote (RFQ) and an itemized list by L-Code. The Contractor warrants the artificial limb/brace provided against defective material, workmanship and fit for a minimum of 90 calendar days from the date of acceptance. The Contractor shall immediately notify all affected VAMC (s) and contracting activities in the event of a product recall, removal, required adjustment, or required modification that is suggested or mandated by the Contractor, distributor, manufacturer, or any regulatory or official agency. The Contractor agrees that by accepting a VISN 8 patient for care, it has available sufficient qualified staff capable of providing quality care necessary to meet the vision of the VISN 8 VA Healthcare System, which is to honor American veterans by providing exceptional healthcare that improves their health and well-being. To assure continuity of care for our patients, the following timelines and customer service standards will be adhered to when treating VISN 8 patients (Reference VHA Directive 1003, VHA s Veteran Customer Service Program, dated August 6, 2003; VHA Handbook 1003.1, Key Elements of VHA s Veteran Customer Service Program, dated August 6, 2003; VHA Handbook 1003.2, Service Recovery in the Veterans Health Administration, dated February 4, 2004; and VHA Directive 1230, Outpatient Scheduling Processes and Procedures, dated July 15, 2016.). Patients will be seen within 20 minutes of scheduled appointment time Patients will have an appointment with a specialist, when referred from VISN 8, within 30 days of referral The Government will not assume any costs incurred by the contractor in the preparation of responses. All responses shall be submitted in writing no later than July 1, 2019, 4:00 PM, EST to the attention of Leyla Campbell at leyla.campbell@va.gov, and Cory Summerville at cory.summerville@va.gov.
