Description
11 The VA Iliana Healthcare System, 1900 East Main Street, in Danville Illinois is seeking sources with the capability to provide vehicles(to include all fuel costs), containers, labor, equipment and disposal for solid waste removal services at the VA Iliana Healthcare System in Danville Illinois. The NAICS code for the solicitation to be released is 562111, with a size standard of $38.5 million. Service Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) Contractor(s) shall respond with their capabilities and proof of registration in VIP at https://www.vip.vetbiz.va.gov/Home/Index. Contractors shall also provide their DUNS number, business size and socioeconomic status under the NAICS Code listed above. All offerors must be registered in the System for Award Management (SAM) database at www.sam.gov prior to submission of any offer. Responses shall be due by 4PM CDT on Friday June 28,2019. Requirement information is as follows: PERFORMANCE WORK STATEMENT Scope of work: Contractor shall provide all vehicles(to include all fuel costs), containers, labor, equipment and disposal for solid waste removal service at the VA Iliana Healthcare System in Danville Illinois. The vendor shall include in their quotation containers and services to include all disposal fees for waste removal services for a base year plus four (4) option years for the VA lliana Healthcare System at 1900 East Main Street, Danville, II 61832 Services to be provided: The vendor shall provide an enclosed 40 cubic yard waste container configured to attach to the VA trash compactor. The unit is to be emptied twice weekly on Tuesdays and Fridays. Provide a 30 cubic yard waste container to be permanently located adjacent to the old scale house with service to empty and return on a will call basis with vendor dispatch. Provide additional 30 cubic yard waste containers for in house projects at various locations on campus as needed on a will call basis arranged with dispatcher. Provide thirty (30) individual 96-gallon toters to quarters and outlying building locations as specified below and provide onsite waste removal service three (3) times weekly Monday, Wednesday and Friday. Building 131 Valor House (5) Commencing with Option Year 1 (1 Oct 2020) the following requirements will need to be added for the duration of the option years covered under the contract. Building 140 4 cubic yard (1) Monday/Thursday Building 132 96 gal toter (5) Mon/Wed/Fri Building 133 96 gaI toter (5) Mon/Wed/Fri SITE ACCESS REQUIREMENTS All personnel working on this project will be required to sign in daily at Police Service located inside the front entrance of Building 102 and obtain a visitor pass to be conspicuously worn while on the installation All personnel working on VA property are required to complete VA Privacy Training and comply with VA Handbook 6500.6, Appendix C, Sections 1,6,7,9a(3) and 9b. Contact COR or POC's listed below for assistance. All onsite personnel on VA projects are required at a minimum to be in possession of a Ten (10) hour OSHA card. Copies to be provided to the COR prior to work commencing 4. Vendor shall coordinate with Carl Wise, VA Engineering {217) 554-5886 or (217) 799-6979 for any deviations from the schedule. Time period Waste removal shall be accomplished per the performance work statement for the base year plus four (4) option years commencing with the first scheduled pickup day after 1 October 2019.
