- Notice Type: Posted Date: Sole Source Notification June 17, 2019 Original Set Aside: Response Date: Intent to Sole Source June 21, 2019 @ 1:00 p.m. EST Classification Code: NAICS Code: J065 Equipment Maintenance and 811219 Other Electronic and Precision Repair Services Equipment Repair and Maintenance Intent to Sole Source: The Department of Veterans Affairs, Network Contracting Office 8; hereby provides a notice of its intent to award a firm fixed contract to provide MAINTENANCE AND REPAIR SERVICE FOR FIFTEEN (15) BARD ® CRITICORE ® MONITOR UNITS located at the Bruce W. Cater VA Medical Center, 1201 NW 16TH Street, Miami, Florida 33125 to the following Contractor. C.R. Bard Inc. 8195 Industrial Blvd Covington, GA 30014 This requested Repair Service is to provide all labor, parts, and transportation necessary to repair fifteen (15) units and bring them all back to manufacture specifications. These units are required to be shipped to the Contractor for replacement of the Original Equipment Manufacturer (OEM) parts and returned fully operational. The following reports are required to be provided for each unit upon completion of repair and return to the facility. Field Action Report Evaluation Report Functional Test Report ANY COMPANY WHO MAY BE ABLE TO PROVIDE NECESSARY ORIGINAL EQUIPMENT MANUFACTURER (OEM) MAINTENANCE AND REPAIR TYPE SERVICE FOR THESE SYSTEMS UNITS MUST PROVIDE PROOF OF AUTHORITY ALLOWING WORK TO BE PERFORMED ON THIS EQUIPMENT, CAPABILITY STATEMENT TO DEMONSTRATE SERVICE ABILITY, AND ANY TYPE GOVERNMENT CONTRACT INFORMATION IF APPLICABLE TO THIS TYPE OF SYSTEM AND REPAIR. ALL EQUIPMENT MUST MAINTAIN THEIR WARRANTY STATUS AND BE FULLY OPERATIONAL AT ALL TIMES FOR PATIENT UTILIZATION REQUIREMENTS. NO GRAY MARKET OR REFURBISHED REPLACEMENT PARTS WILL BE ACCEPTABLE, MUST HAVE ORIGINAL EQUIPMENT MANUFACTURER (OEM) PARTS. Any prospective candidates must be registered in the System for Award Management (SAM) in order to be eligible for award. All qualified SDVOSB/VOSB candidates must be verified in Vendor Information Pages (VIP) in order to be eligible for award. If you have not conducted business with the Bruce W. Carter VA Medical Center located in Miami Florida, you will be required to complete Vendor/Payment Forms to be entered into the local systems in order to be eligible for award. All interested parties shall submit clear and concise documentation demonstrating their capabilities to satisfy the requirements listed above to Contracting Specialist, Cynthia Machuca at cynthia.machuca@va.gov.
