THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR QUOTE. The Department of Veterans Affairs, NCO 12, Great Lakes Acquisition Center, 115 s 84th street, Suite 101, Milwaukee, WI 53225 is issuing this sources sought synopsis as a means of conducting market research to identify parties having an interest in, and the resources to support the acquisition and installation of decorative wall panels for 3 common areas of the Edward Hines JR VA Medical Center in Hines, IL. The purpose of this Sources Sought notice is to determine interest and capability of potential vendors, including Small Business, Veteran-Owned Small Business (VOSB), and Service-Disabled Veteran-Own Small Business (SDVOSB) vendors relative to the North American Industry Classification System (NAICS) Code 327390- Architectural wall panels, precast concrete, manufacturing THERE IS NO SOLICITATION AT THIS TIME. This request for capability information does not constitute a request for proposals/quotes; submission of any information in response to this market survey is purely voluntary; the government assumes no financial responsibility for any costs incurred. If your organization has the potential capacity to perform these contract requirements, please provide the following information: 1) Organization name, address, email address, Web site address, telephone number, and size and type of ownership for the organization; and 2) Tailored capability statements addressing the particulars of this effort, with appropriate documentation supporting claims of organizational and staff capability. The government will evaluate market information to ascertain potential market capacity to provide services consistent in scope and scale with those described in this notice and otherwise anticipated. BASED ON THE RESPONSES TO THIS SOURCES SOUGHT NOTICE/MARKET RESEARCH, THIS REQUIREMENT MAY BE SET-ASIDE FOR SMALL BUSINESSES OR PROCURED THROUGH FULL AND OPEN COMPETITION. Submission Instructions: Interested parties who consider themselves qualified to perform the below-listed services are invited to submit a response to this Sources Sought Notice by 12:00 PM CST on 01 July, 2019. STATEMENT OF WORK (SOW) -INTERIOR DESIGN ACQUISITIONS HINES VA HOSPITAL (578) Address: Edward Hines VA Hospital, 5000 S. 5th Ave., Hines, IL. 60141 1. Objectives: Provide patient decorative panels for wayfinding improvements for patient CafĂ©, Market, and Atrium interior entrances. 2. Contract Title: Decorative Wall Panels -patient convenience areas building 1 - outer corridor 3. Background: The building is approx. 100 years old, providing decorative wall panels over the existing brick will provide points of interest for the patients to find the dining, market, and atrium areas. 4. Scope Manufacture, Deliver and Install per statement of work and exhibit design plans. 5. Specific Tasks Manufacture, Deliver and install the following product listed below. Project installation is to be coordinated, so that patient care is minimally interrupted. Product is to be installed in a max. of 3 phases to allow for patient access in the corridor. If installers can complete in fewer phases, present the option for review. Phasing must accommodate patient care access and be completed off hours. Work will need to be performed on a Saturday between 7am and 3pm. Minor staging/calculating of the wall area can be done during normal business hours, if it does not interfere with traffic flow or patients access. General Notes: Pre-bid walk through can be coordinated if necessary, per the contracting officer. Field verify all panels sizes and mounting requirements, prior to purchase and install Field verify any obstruction to cut around in the field, prior to purchase and install All panel cut outs are to be verified and field fit, with aluminum trim edging Edges are to be free of any sharp edges. No exposed partical board will be accepted Submit install plan drawings and product material/MSDS sheets for review and sign off by COR Install : Panels are to be unfinished with aluminum thin trim edge and field fit Install is to be demountable (removaable). Adhesive application is not acceptable. Method for install is to be security mounted and concelaed behind panles per manuafactures installation instructions. Locking snap product with suction cup install metod, per manufactures instrucions for demountable install Panels are to be installed with grain of wood running horizontal Side panels that are runnning vetrtical should have grain match vertical direction Drilling method must include dust free vacuuming attachment while drilling All dimensions are to be field verified at the site walkthrough prior to purchase of material Color match filler caulk at top and bottom and exposed edges of panels/walls Secure new base to existing terrazzo base with metal fasteners per manufacture Base is 6 high, existing base is approx. 4.5 high, and projects ľ -1. Wall panels are to be installed starting at top of existing base behind wall base panel. Wall panel bottom edge to be install at top of base and behind new 6 base providing extra wall protection Any discrepancies and dimensions should be reviewed prior to install with COR. All exposed panel edges are to be covered with the aluminum trim Where new corner guards are being provided, new decorative panels abut corner guard Reveals are to be thin trim aluminum edge detail between panels Butt panels to trim, provided by manufacture Existing door frames and trim will remain and be painted by owner Set up caution cones/flags and tape for install, only 3 feet max of corridor can be block at install of panels Basis of Design: Decorative wall panel system to be by Acrovyn Wall Panel or equal, with factory bonded rigid wallcovering and unfinished edge to include aluminum thin trim 1/16 factory installed on panel. Engineered PETG wall panels with manufactured of.040 thick Acrovyn 4000 Chameleon Simulated patterns factory bonded to fiber core or equal. Suede texture factory bonded to face and sides of 3/8 thick particle board core assembly. Back side of panel is to be laminated for moisture barrier. Overall panel thickness is approx. 1 max with demountable Sure Snap locking or Equal. Mounting system hardware mechanically fastened to wall and preinstalled on panel, using suction cup install and metal fasteners. Coordinating products to match system are the crash rails and faux brushed nickel panels. Wall base is by Acrovyn or equal. Saratoga Acrovyn wrapped with Chameleon wrapped over wood base, or equal. Performance Monitoring - On site review and punch list will be conducted, for accurate install of product. Phasing schedule is to be coordinated with COR, for scheduling with in contract time. 7. Security Requirements Manufacture will be badged by PIV and escorted on campus to site of install. No VA Sensitive information or connection to the Hines network will be required for execution of this contract. 8. Other Pertinent Information or Special Considerations. Packaging, Packing and Shipping Instructions - Deliver to warehouse building 220 Hines Campus. If Items need to be acclimated at site coordinate with COR for access to adjacent space for panels. 9. Risk Control - N/A 10. Place of Performance: Furniture/product is to be delivered to the Warehouse BLDG 220 at Edward Hines VA Hospital, 5000 S. 5th Ave., Hines, IL 60141. Work is to be performed during off hours, Saturday 7am 3:30. 12. Period of Performance - 60 Days from receipt of order. Deliveries must be coordinated and scheduled in advance. 13.
