Description
1. This is a combined synopsis/solicitation for commercial services/items prepared in accordance with the format in FAR Subpart 12.6 in conjunction with FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. a. Project Title: Re-Core Door Locks b. Description: VA Loma Linda Healthcare System is seeking a contractor to replace door lock cores and provide new keys for locks throughout the medical center. c. Project Location: VA Loma Linda Healthcare System, 11201 Benton Street, Loma Linda, CA 92357 d. Solicitation Type - Number: Request for Quotes (RFQ) - 36C26219Q0837. e. Set Aside: This solicitation is 100% set aside for Service Disabled Veteran Owned Small Businesses IAW Public Law 109-461, 38 USC Section 8127 f. Applicable NAICS code: 561622, Locksmiths g. Small Business Size Standard: $20.5 Million h. Type of Contract: Firm Fixed Price i. Period of Performance: 180 days after notice to proceed 2. Statement of Work 2.1 INTRODUCTION. The requirement is to re-key doors in Veterans Administration Loma Linda Medical Center (VALLMC). 2.2 BACKGROUND / DESCRIPTION OF WORK. The Veterans Affairs Healthcare System, located in Loma Linda, California (605) requires contractor services to replace door cores on VALLMC Main Hospital, Pharmacy Building #20, Auditory Building #30, Cancer Center Building #31, and the Valor Building #32. 2.3 REQUIREMENTS. 2.3.A. Qualifications. Contractor shall have a minimum 5 years locksmith experience and provide references for three similar jobs within the past three years. 2.3.B. General Requirements 2.3.B.1. The contractor shall provide all management, labor, material, equipment, transportation, supervision to accomplish the installation of new door cores and keys in the Veterans Administration Loma Linda Medical Center (VALLMC) Main Hospital, Pharmacy Building #20, Auditory Building #30, Cancer Center Building #31, and the Valor Building #32. 2.3.B.2. Prior to core installation the contractor shall conduct a survey of the entire VALLMC complex and other required buildings. Contractor will provide a door schedule for each facility organized by floor. Schedule will include facility number, door number, core number assignment, core keyway, if door is equipped with a PIV locking system, room function and service the room belongs. Schedule shall be created in Microsoft excel or Microsoft access only and must be approved by VALLMC contracting office representative (COR) before core installation can commence. 2.3.B.3. Core assignments will follow provided VALLMC key hierarchy. 2.3.B.4. Contractor shall provide a core installation schedule for all cores to COR for approval 30 days prior to installation of first core. Core installation schedule will be organized by hospital service. Any deviations to approved schedule will require approval by COR. COR shall be notified in writing within 5 days after completion of a service core installation. 2.3.B.5. Core installation will not begin until all cores for the hospital have been received, pinned to approved hierarchy, keys cut and keys issued to service staff. 2.3.B.6. Contractor to provide cores for all doors on VA approved door schedule. Cores shall be BEST CORMAX hardware to match new key system at VALLMC and ACC. No other key product or brand is acceptable. Contractor shall provide 4070 cores that will require pinning to door schedule hierarchy. Contractor shall also provide 75 additional blank cores to be used and configured to Nursing Service. 2.3.B.7. Contactor shall cut two (2) keys per core. Keys shall be stamped with Do Not Duplicate on one side with key number and unit of issue number on the other. Unused key blanks shall be turned over to VALLMC Police Locksmith at the completion of the contract. 2.3.B.8. Contractor shall issue keys to hospital staff according to the following schedule (Mon thru Friday 0700-1000 and 1300-1500) one week prior to a services core installation. VA will provide a space for contractor s use, to issue keys. VA will provide a computer and signature pad for contractor s use, to issue keys. Unissued keys will be turned into VA Police the following Monday after the last day of the issue period for that service. 2.3.B.9. Documents are for reference only, it is the contractor s responsibility to field verify information provided. Approximate door count for all facilities is 4,070. 2.4. HOURS OF OPERATION. 2.4.A. Normal hours of operation are Monday through Friday from 7:00 am to 4:00 pm (PST). All work is expected to be performed generally during normal hours of coverage unless requested or approved by the COR or his/her designee. Core installation shall be conducted in the evening hours between 4:00 pm and 11:00 pm 2.4.B. Before commencement of work, the Contractor shall coordinate with the VALLMC COR and Contracting Office to agree on a sequence of procedures; means of access to premises and building. All work performed, to include delivery of materials and equipment, shall be made with minimal interference to Government operations and personnel. 2.4.C. Work performed outside the normal hours of coverage must be approved by the COR or his/her designee. 2.4.D. Federal Holidays observed by VALLMC are: New Years' Day Labor Day Martin Luther King Day Columbus Day Presidents' Day Veterans' Day Memorial Day Thanksgiving Day Independence Day Christmas Day 2.5. IDENTIFICATION, PARKING, SMOKING, AND VA REGULATIONS. 2.5.A. The contractor shall comply with security regulations imposed by the VALLMC Chief of Police or their designated representative and the agency occupying the space where work is to be performed, including any necessary security clearances. 2.5.B. The Contractor's employees and sub-contractors shall wear visible VA provided identification at all times while on the premises of VALLMC. Cards may be issued for the period of performance of the contract only or one year whichever is a shorter period of time. Contractor personnel are responsible for safe guarding identification cards issued at all times. Any lost or stolen cards shall be reported to the COR immediately. 2.5.C. Only one contractor vehicle will be authorized to park on property nearest to the work site. Additional vehicles must be requested through the VA Chief of Police or designee for approval. No contractor s personal vehicles will be authorized to park on-property. Suggested off-site parking areas for contractor s personal vehicles are: Barton Road, East Prospect, and Benton Street. 2.5.D. To ensure unimpeded access for firefighting equipment, ambulances and delivery service vehicles, parking is prohibited in the following locations at all times. Emergency driveway and unloading area Driveways leading to the Medical Center, loading platforms, and fire zones Roadways where parking would interfere with the free flow of traffic. This includes the bus lanes in the front circle of the Medical Center Within 15 feet of a fire hydrant (curb or street side of hydrant only) All VA Police barricaded/coned off areas. NOTE: Only VA Police Officers are authorized to barricade/cone off designated areas, unless approval by the VA Chief of Police is otherwise granted. The contractor shall not drive any construction equipment with metal wheels or tracks (i.e. roller, excavator etc.) on the streets of VALLMC. All equipment shall be loaded on a trailer and hauled to the location. 2.5.E. There shall be no tobacco use inside VALLMC. This rule applies to all facilities, whether occupied or not, under construction/renovation or not. Cigarettes butts shall not be left on the ground at any construction site. The contractor shall strictly inform/enforce these rules regarding his employees, suppliers, and subcontractors. 2.5.F. The contractor assumes responsibility for the actions of all his/her employees, subcontractors, and suppliers when on VALLMC property. 2.5.G. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court. 2.5.H. The contractor shall assume full responsibility and liability for compliance with the applicable regulations pertaining to the health and safety of personnel during the execution of work and shall hold the Government harmless for any action on his part or that of his employee or subcontractors, which results in illness, injury or death. 2.5.I. The contractor shall establish and maintain a safety plan to ensure the safety requirements of the contract are provided as specified. The safety plan shall be provided as specified and submitted to the CO within 30 calendar days after contract award. The safety plan shall be subject to Government approval. Contractor compliance with the safety plan will be monitored by the Government. 2.5.J. The contractor must have OSHA safety certification training to work on hospital property. OSHA 30-hour safety training course or OSHA recognized equivalent will be required for all job site supervisors, project managers, superintendents and foremen. OSHA 10-hour safety training or OSHA recognized equivalent will be required for all other job site employees and sub-contractor employees. The certifications must be included in the contractor s safety plan. 2.5.K. The contractor shall comply with all applicable state and local laws, ordinances, VALLMC policies and regulations relative to noise control and infectious control. The Government may require that operations that generate excessive noise be scheduled at other than standard work hours. 2.5.L. The contractor shall obtain a burn permit prior to conducting any burning or welding. Copies shall be submitted to the COR and posted at the jobsite. All fire and safety regulations are to be followed stringently. 2.5.M. All work in secure areas shall be under escort by Government personnel. The VA Police may require a local agency check of all personnel involved in the accomplishment of the work prior to commencement of work 2.6 INFORMATION SECURITY. In accordance with Appendix A, Handbook 6500.6 Checklist blocks 7, the C&A requirements do not apply to this requirement, and Security Accreditation Package is not required. The nature of this requirement is technical; however, there is no information to be protected. 2.7 OTHER PERTINENT INFORMATION OR SPECIAL CONSIDERATIONS. 2.7.A. Access to worksite shall be coordinated through the Facilities Management Section at: Loma Linda VA Health Care System (605) 11201 Benton Street Loma Linda, California 92357 Point of Contact COR Wendy Zamora (909) 825-7084, ext 1400 2.7.B. All contractor equipment is the sole responsibility of the contractor. The Government is not liable for the theft, vandalism, or damage to any contractor supplies, equipment or property. The contractor shall secure his materials and protect all equipment from damage 2.7.C. Any equipment/material the contractor removes, and which requires reinstallation as part of the job requirements shall be the responsibility of the contractor. All damaged material shall be replaced with equipment/material of equal quality and function by the contractor at his own expense 2.9. PERIOD OF PERFORMANCE. Commencement of work shall begin within 10 days after receipt of written Notice to Proceed (NTP) and shall be completed within 180 days. 2.10. COMMENCEMENT, COMPLETION, AND PAYMENT FOR WORK. 2.10.A. Before commencement of work, the Contractor shall confer with the VA COR and Contracting Office to agree on a sequence of procedures; means of access to premises and building; space for storage of materials and equipment; delivery of materials and use of approaches; use of corridors, stairways, elevators, and similar means of communications; location of partitions, eating spaces, and restrooms for the Contractor employees; and the like. Delivery of materials and equipment shall be made with minimal interference to Government operations and personnel. 2.10.B. At the conclusion of this contract, the contractor shall be required to vacate the areas provided to him including office area and individual worksites. The contractor shall return the office area to its original condition and shall do so within 14 calendar days of being notified by the CO, or the conclusion of this contract, whichever comes first. 2.10.C. The contractor shall provide a minimum of one-year warranty on all work. The COR will notify the contractor of warranty issues. After being notified of a problem, the contractor shall investigate the issue and respond within 24 hours. If the contractor believes the issue is not warranty related, he may request a fact-finding visit with the CO and COR. If the fact-finding meeting determines the issue is warranty related, the contractor shall repair the item within 48 hours, or as determined by the COR. The contractor is responsible for ensuring subcontractors fulfill warranty requirements on their work. All manufacturer warranties for equipment, etc. installed shall be submitted to the CO. 2.11 MODIFICATIONS. 2.11.A. Modifications for this project after award will utilize the modification table and will be filled out when submitting a request. All requests must be submitted to the assigned COR who will then submit a formal request to the Contracting Officer (CO). New work is not allowed to commence unless it is signed by both parties via a Standard Form (SF) 30 modification. 3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-02, dated 5/6/2019. The following provisions and clauses apply to this solicitation and can be found in enclosure 1. a. FAR 52.212-1, Instructions to Offerors Commercial Items b. FAR 52.212-2, Evaluation -- Commercial Items c. FAR 52.212-4, Contract Terms and Conditions -- Commercial Items d. FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items 4. Key solicitation milestones are: 4.1. A site survey will be conducted at 10am on Friday, June 28, 2019. Participants will meet near the Starbucks at the main entrance to the Medical Center. No questions will be answered after this date/time unless determined to be in the best interest of the Government as determined by the Contracting Officer. Telephonic questions will not be accepted or returned. RFI responses will be responded to as necessary in amendment format which will be posted on www.fbo.gov. 4.3. Submit written offers in accordance with the following instructions, as well as Addendum to FAR 52.212-1, Instruction to Offerors. Oral offers will not be accepted. Complete and submit enclosure 2 via email no later than 10am PDT Tuesday July 17, 2019 to peter.kim33@va.gov. Ensure the following are completed and/or included in the quote packet: a.
