This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. The associated North American Industrial Classification System (NAICS) code for this procurement is 334510, Electromedical and Electrotherapeutic Apparatus Manufacturing, with a small business size standard of 1,250 employees. This is a 100% total Service Disabled-Veteran Owned Small Business set-aside procurement. The Aleda E. Lutz VA Medical Center, 1500 Weiss Street, Saginaw, Michigan 48602, is seeking to procure a Lumenis Laser Moses Upgrade, that will include installation, calibration, and on-site training. Moses Upgrade to P120H & 4-year Ultimate Warranty Bundle includes: Moses Technology Upgrade to standard Pulse 120H Moses Technology Software with SIS fiber recognition technology, 4 Moses Bricks, Moses board, Moses front panel and onsite installation. Four years of Ultimate Warranty Energy Care Ultimate Plan (NC24), is a comprehensive all-inclusive program that provides the following: Two (2) preventative maintenance (PM) calls per year; all labor and travel; all parts. Machine down is defined as a condition that renders the laser inoperative. Intermittent and limited functionality are not considered machine down. Two (2) business day response time. Response time for HI and AK will be five (5) business days. Access to refurbished parts as available. Installation, Calibration & On-Site Training: In-hospital laser training program for the O.R. nursing staff by a Lumenis Regional Education Manager (qualifies for up to 4.0 contact hours by ANA), must be used within 1 year of shipment. On-Site Services are available Monday-Friday, 8:00am 5:00pm, and subject to availability.
