DEPARTMENT OF VETERANS AFFAIRS NETWORK CONTRACTING OFFICE (NCO) 16 MICHAEL E. DEBAKEY VA MEDICAL CENTER (VAMC), HOUSTON, TEXAS REQUEST FOR INFORMATION 36C25619Q01086 BACKGROUND: The Houston VAMC has a requirement for the following BRAND NAME OR EQUAL supplies: Manufacturer Stock # Item Description Please see SOW attached REQUESTED RESPONSES: The intent of this Request for Information is to establish sources to define the procurement strategy (e.g set-aside, sole source, unrestricted) for a solicitation that VA intends to post in the near future. Interested contractors are requested to respond in accordance with the following: Please respond to this RFI if you can provide the exact brand name products listed in the table in the background section above. In the response please cite your business size status. If you have an existing GSA or VA, Federal Supply Schedule contract, please include the contract details in your response. Please respond to this RFI if you can provide supplies that can be determined EQUIVALENT to the products listed in the table in the background section above. Please provide details on the proposed EQUIVALENT products such as Manufacturer Name, Part Number, and Description. If you have an existing GSA or VA, Federal Supply Schedule contract, please include the contract details in your response. Please note that VA is particularly interested in determining the availability of Small Business Manufacturers. Respondents are solely responsible for all expenses associated with responding to this RFI. STATEMENT OF WORK 05/22/2019 580-19-3-8103-0158 A. GENERAL INFORMATION 1. Title of Project: Purchase of one (1) Ultra Low Temperature Upright Freezers Model # FDE60086FA; one (1) Shelf kit; Thermo Scientific; Includes: 6 sliding drawer racks and (150) 2 in. boxes with 100 cell dividers; For use with Forma 88600; one (1) Shelf kit; Thermo Scientific; Includes: 6 sliding drawer racks and (150) 2 in. boxes with 100 cell dividers; For use with Forma 88600. 2. Project Scope: Provide equipment to be shipped to: Baylor College of Medicine Dept. of Pathology One Baylor Plaza - Room T207 Houston, TX 77030 Atten: Drs. M. Ittmann and P. Castro The contractor shall provide all resources necessary to accomplish the deliverables described in the statement of work (SOW), except as may otherwise be specified. - One (1) Ultra Low Temperature Upright Freezers Model # FDE60086FA - One (1) Shelf kit; Thermo Scientific; Includes: 6 sliding drawer racks and (150) 2 in. boxes with 100 cell dividers; For use with Forma 88600 - One (1) Shelf kit; Thermo Scientific; Includes: 6 sliding drawer racks and (150) 2 in. boxes with 100 cell dividers; For use with Forma 88600. 3. Background: Research Investigators will be utilizing the above-named equipment in support of their ongoing research project. 4. Performance Period: The period of performance will be determined by the delivery date of the equipment to MEDVAMC. 5. Type of Contract: Firm-Fixed-Price 6. Place of Performance: Baylor College of Medicine Dept. of Pathology One Baylor Plaza - Room T207 Houston, TX 77030 B. CONTRACT AWARD MEETING No contractor award meeting will be held. C. GENERAL REQUIREMENTS 1. For every task, the contractor shall identify in writing all necessary subtasks (if any). 2. All written deliverables will be phrased in layperson language. Statistical and other technical terminology will not be used without providing a glossary of terms. D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Description of Tasks and Associated Deliverables: The contractor shall provide the specific deliverables described below within the performance period stated in Section A.4 of this SOW. Task One: Provide equipment. Deliverable: o One (1) Ultra Low Temperatre Upright Freezers Model # FDE60086FA o One (1) Shelf kit; Thermo Scientific; Includes: 6 sliding drawer racks and (150) 2 in. boxes with 100 cell dividers; For use with Forma 88600 o One (1) Shelf kit; Thermo Scientific; Includes: 6 sliding drawer racks and (150) 2 in. boxes with 100 cell dividers; For use with Forma 88600. E. SCHEDULE FOR DELIVERABLES 1. The contractor shall complete the Delivery Date column in Attachment A for each deliverable specified. 2. Unless otherwise specified, the number of draft copies and the number of final copies shall be the same. 1. If for any reason the scheduled time for a deliverable cannot be met, the contractor is required to explain why (include the original deliverable due date) in writing to the CO, including a firm commitment of when the work shall be completed. This notice to the CO shall cite the reasons for the delay, and the impact on the overall project. The CO will then review the facts and issue a response in accordance with applicable regulations. F. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. J. GOVERNMENT RESPONSIBILITIES The government will provide controlled access to the areas and other resources if required to perform the services. The government will assign a primary POC, and to provide information and resources in a manner to maintain contract continuity. Additionally, this person will receive deliverables as a result of this contract.
