Description
3 Please note that this is NOT a request for quotes or proposals, the Government is seeking information for market research purposes only. VHA Reginal Procurement Office (RPO) 2 is currently conducting a market survey for qualified firms capable of providing Planmeca Prosensor HD Sets. The North American Industry Classification Systems (NAICS) is 339114. Responses to this notice must be submitted in writing (email or fax) and must be received no later than July 10, 2019 3PM EST. Email: LATEISHA.ROBINSON@VA.GOV and Fax: 718-741-4722. No telephone inquiries will be accepted. Interested parties MUST provide company/individual name, a capability statement, examples of same or similar work performed at other facilities, DUNS number and address, point of contact and social-economic. Interested parties who do not submit the above information on or before the response due date and time will not be considered for this procurement. Please review the attached Statement of Work attached herein. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, women owned small business, HUB Zone, 8(a), small business or large business), relative to NAICS 339114. Responses to this notice will be used by the Government to make appropriate acquisition decisions. A solicitation is not currently available. If a solicitation is issued, it will be announced on GSA E-buy website http://www.fbo.gov at a later date, and all interested parties must respond to that solicitation STATEMENT OF WORK GENERAL INFORMATION 1. Title of Project: PLANMECA ProSensor HD 2. Scope of Work: The James J. Peters VAMC Dental Program is seeking replacement of X-rays dental sensors deemed obsolete for the Dept. to use as per Biomed. The recommended replacement is for the dental FE003793ETH-K PLANMECA ProSensor HD Set with Sensor Size 2. Planmeca ProSensor ® HD is ergonomically designed with 3 separate sizes (0,1 & 2) this item has a Magnetic sensor connection for one-handed use which is integrated to a traditional technique with Durable Kevlar shielded cable with only 2 wires. This item has a LED light color-code alert to inform the operator of the connection, it true resolution of over 22/lp.mm has a wide dynamic range it is compatible with Windows and Mac OS. This is utilized for infection control. The measurement of each dimension are as follows: Pixel size 15 m x 15 m Sensor dimensions Size 0 overall 33.6 x 23.4 mm (1.33 x 0.92 in.) active area 25.5 x 18.9 mm (1.00 x 0.74 in.) pixel matrix 1700 x 1258 number of pixels 2.14 M Size 1 overall 39.7 x 25.05 (1.56 x 0.99 in.) active area 30.6 x 20.7 (1.20 x 0.81 in.) pixel matrix 2040 x 1380 number of pixels 2.82 M Size 2 overall 44.1 x 30.4 mm (1.74 x 1.2 in.) active area 36 x 26.1 mm (1.42 x 1.03 in.) pixel matrix 2400 x 1740 number of pixels 4.18 M 3. Contract Type: Firm-Fixed-Price as per GSA contract # 36F79719DO106 The contractor shall complete the work required under this SOW in 60 calendar days or less from date of award, unless otherwise directed by the Contracting Officer. If the contractor proposes an earlier completion date, and the Government accepts the contractor s proposal, the contractor s proposed completion date shall prevail. B. CONFIDENTIALITY AND NONDISCLOSURE It is agreed that: 1. The preliminary and final deliverables and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this task order are the exclusive property of the U.S. Government and shall be submitted to the Contracting Officer at the conclusion of the task order. 2. The Contracting Officer will be the sole authorized official to release verbally or in writing, any data, the draft deliverables, the final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order presented to the contractor shall be submitted to the Contracting Officer for response. 3. Press releases, marketing material or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the Contracting Officer.
