This announcement constitutes the only solicitation; quotes are being requested. This is an unrestricted solicitation. Quillen VAMC 205 Bradford Pear Lane Mt. Home, TN 37684 ALL DELIVERIES WILL BE MADE FOB DESTINATION. To be responsive and thereby considered, all quoted items must be the requested brand name specified in this RFQ. Award will be made in the aggregate, all or none. Be sure to read addendums for FAR 52.212-1 and FAR 52.212-2. Failure to follow guidelines as set forth in this solicitation could lead to exclusion due to non-responsiveness of offer. SOLICITATION PROVISIONS Clauses included in this combined solicitation/synopsis are not all inclusive. Clauses included on any ensuing award may include all or some of the below clauses. Contractor s should assume that all the below clauses are applicable to this acquisition. Additional clauses may also be added at the discretion of the contracting officer upon award 52.212-1 INSTRUCTIONS TO OFFERORS-COMMERCIAL ITEMS (IAW FAR 12.301(b)(1)) AUG 2018 Addendum to 52.212-1: Paragraph (b), entitled Submission of Offers, : Offers may be submitted via handcarry or electronic mail. Submit signed and dated offer at or before 12:00 PM PST, 3 JUN 2019. To send offer via electronic mail all documents must be scanned in.pdf (Acrobat) format and emailed to wyatt.williams@va.gov. Submit quotations on company letterhead or pricing document. Quotations must include as a minimum: company contact information, price breakout for the requested line items, CAGE Code, a completed FAR Provision 52.212-3, Representation and Certification with a copy of a verification of registration with the System for Awards Management (http://sam.gov/SAM/), and any other information relevant to the proposal. No telephonic responses will be processed. The Government reserves the right to make award on the initial quote without discussions. (End of Clause) A.3 VAAR 852.211-70 EQUIPMENT OPERATION AND MAINTENANCE MANUALS (NOV 2018) The Contractor shall follow standard commercial practices to furnish manual(s), handbook(s) or brochure(s) containing operation, installation, and maintenance instructions, including pictures or illustrations, schematics, and complete repair/test guides, as necessary, for technical medical equipment and devices, and/or other technical and mechanical equipment provided per CLIN(s). The manuals, handbooks or brochures shall be provided in hard copy, soft copy or with electronic access instructions, consistent with standard industry practices for the equipment or device. Where applicable, the manuals, handbooks or brochures will include electrical data and connection diagrams for all utilities. The documentation shall also contain a complete list of all replaceable parts showing part number, name, and quantity required. (End of Clause) A.6 VAAR 852.273-74 AWARD WITHOUT EXCHANGES (JAN 2003) The Government intends to evaluate proposals and award a contract without exchanges with offerors. Therefore, each initial offer should contain the offeror's best terms from a cost or price and technical standpoint.
