- The USDA Forest Service (FS) Washington Office (WO) of Acquisition Management (AQM) is seeking information on qualified Contractors able to provide services in support of the WO Employee Wellness Program managed by the Office of Safety and Occupational Health (OSOH). THIS IS A REQUEST FOR INFORMATION (RFI) ONLY. This RFI is issued solely for information and planning purposes. This is NOT a solicitation for proposals or quotations. The Government will not acccept unsolicited proposals in response to this notice. No reimbursement will be made for any costs associated with providing information in response to this RFI. Responses to this request will not be considered adequate responses to a solicitation. Not responding to this RFI will not exclude a prospective vendor from participation in any future RFP/RFQ, if issued. No proprietary, classified, confidential, or sensitive information shall be included in responses to this RFI. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Respondents are advised that the Government is under no obligation to acknowledge receipt of the inforamtion received or provide feedback to respondents with respect to any infromation received. Please reference the attached DRAFT statement of work (SOW) for further details on this requirement. Information may be submitted regarding the draft SOW, including feedback concerning any areas of support that may be missing or inadequately defined. The Government requests that interested parties: 1. Assist in identifying if the requirement is inadequately or ambiguously defined; 2. Suggest information or data that the Government could provide to improve the likelihood of obtaining accurate pricing; 3. Provide feedback on contract type (e.g. firm fixed-price); and, 4. Provide any other relevant comments, suggestions, or feedback. The Government also requests that responses include the following administrative and business information: 1. Company name; 2. Company address; 3. Company point of contact and contact information; 4. Company DUNs; and, 5. Company business designation (e.g. small business, 8(a). General questions may be submitted via e-mail to renee.jones-chuang@usda.gov no later than 15:00 ET on Friday, July 5, 2019.
