To facilitate the award process, all quotes, must include a statement regarding the terms and conditions herein as follows: The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-03. The associated North American Industrial Classification System (NAICS) code for this procurement is 811198 All Other Automotive Repair and Maintenance, with a small business size standard of 7.5M. The Carl T. Hayden Medical Center 650 E. Indian School Road Phoenix, AZ 85012 is seeking to purchase BOX TRUCK REPLACEMENT OF LIFT DECK AND BODY REPAIR Site Visit (Required): VA Medical Center 650 E Indian School Road Phoenix, AZ 85012-1892 July 8, 2019 @ 9am Reference attached campus map for location. All interested companies shall provide quotation s for the following: BOX TRUCK REPLACEMENT OF LIFT AND BODY REPAIR Carl T. Hayden Medical Center 650 E. Indian School Road Phoenix, AZ 85012 Contents: Scope of Work/Technical & Special Requirements Government Point of Contact Final Payment PRIVACY STATEMENT Scope of Work / Technical Requirements: The scope of work will include the removal of the non-serviceable tailgate lift and the procurement and installation of (1) one tailgate lifts. The scope of work will also include the replacement and repair of damaged body parts of the truck box assembly as needed. Special Requirements The lift must have a minimum capacity of 3000 LBS. The lift must be built using aluminum and will weigh no more than 655 LBS. The lift must have a minimum factory warranty of 2 years from date of initial service. Hayden VA Medical Center 650 E Indian School Road Phoenix, AZ 85012-1892 PRIVACY AND SECURITY STATEMENT: In reference to VHA Handbook 6500.6 Appendix A, Block 6, the equipment included in this contract does not involve connection of IT devices to a VA network. Therefore, C&A, SAP, & the following from App C apply. In reference to VHA Handbook 6500.6 Appendix A, Block 7, the equipment involved does not involve storing, generating, transmitting, or exchanging VA sensitive information.   Therefore, the following from App C apply. (Appendix B) The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information protection, patient privacy, and information system security as delineated in this contract. During contract performance, contractor may require access to VA facility, equipment, information systems, or sensitive data; to provide services, install, train, maintain or repair equipment. We will have contractors involved with this equipment purchase. The delivery/task order period of performance will be determined by contractor. Delivery of truck to contractor shall be provided no later than 30 days after receipt of award of contract. The contractor shall conduct work at contractor location. Hayden VA Medical Center 650 E Indian School Road Phoenix, AZ 85012-1892 INSPECTION AND ACCEPTANCE Inspection and acceptance of the services will be made by the following: Government POC Eric Foster 602-277-5551 x7871 Carl T.Hayden VAMC Logistics Division or its representative. Upon completion of the contract work, the Contractor will furnish documentation, such as a delivery ticket, and warranty documentation immediately following completion of the contract work. The Government will use this documentation to accept services rendered. The Contractor will furnish a copy of the signed acceptance document with its payment requests. BASIS OF AWARD Award shall be made to the quoter whose quotation, offers the best value to the government, considering, technical capability, and price. The government will evaluate information based on the following evaluation criteria: (1) Technical capability factor "meeting or exceeding the requirement, (2) Price.     Offerors must complete annual representations and certifications electronically via the System for Award Management (SAM) website located at https://www.sam.gov/portal in accordance with FAR 52.212-3, Offerors Representations and Certifications Commercial Items. If paragraph (j) of the provision is applicable, a written submission is required. All quoters shall submit the following: Proof of registered business concern. Statement of technical capability to perform all replacements and repairs. Attendance at the conference will be evidenced by the representative s signature on the signature roster. Quoters must bring a copy of the solicitation to the site visit. Any changes resulting from this site visit will be issued in a written addendum to the solicitation.
