Description
This is a Request for Information (RFI) only This (RFI) will close on 07/08/2019 at 10:00am EST. Please complete the Vendor Information Document and email to Wendy.Stewart@va.gov. No phone calls will be accepted. STATEMENT OF WORK JOHN D. DINGELL VA MEDICAL CENTER REGULATED WASTE DISPOSAL FACILITIES COVERED UNDER THIS SOW 1. John D. Dingell VAMC, 4646 John R. Street, Detroit MI 48201 2. 44200 Woodward Avenue, Suite 208, Pontiac, MI 48341 3. 4777 Outer Drive E. Detroit Michigan 48234 REGULATED WASTE 1. The Contractor shall provide (50) red, plastic containers weekly f or removal of regulated medical waste and pathological waste at the John D. Dingell VAMC site and (2) at all other sites. The size/volume of these containers shall be approximately 24'x24'x15' or 28 gallons for the purpose of bulk packaging and shipment. Bulk Shipment Containers must meet VHA, DOT and State of Michigan Requirements the packaging and shipment of medical, pathological and Cytotoxic waste. If quantity increases, pick-ups and container quantities shall be adjusted accordingly. 2. The contractor shall provide shipping quality biohazardous labeled corrugated cardboard boxes for the purpose of shipping sharps containers. These boxes must meet federal and state of Michigan regulations for packaging Biohazardous waste., will allow no more than 50lbs of packaged material. 3. Containers must be reviewed as a part of the bidding process and COR has final selection approval on all containers. 4. Containers and boxes for pathological and chemo waste shall be differentiated from those for regular medical waste to deter confusion among VA employees. 5. The Contractor shall provide a self-sufficient, temperature-controlled trailer for storage of medical waste. The VA will monitor and control the temperature in the trailer. Offsite Regulated Medical Waste will be held on-site at those locations until picked up by the Contractor. 6. For each pick-up of regulated medical waste, the Contractor shall provide a complete copy of the shipping papers for the transportation and disposal of said regulated medical waste. The shipping papers shall at least contain the Generator Copy of the manifest at the time when the medical waste is picked up. The Contractor shall then provide the VA a Generator Copy of the manifest signed by the disposal facility operator(s) within two (2) weeks after the medical waste is delivered to the waste disposal facility. The Contractor shall submit this paperwork within the established two (2) week timeline 95% of the time. 7. The Contractor shall comply with all Local, State, and Federal regulations for the disposal of all medical waste materials. Regular medical waste can be autoclaved, and land filled. However, all pathological waste shall be incinerated. 8. The Contractor shall ensure the Contractor's employees providing work on this contract are fully trained and completely competent to perform the required work. 9. The Contractor shall provide a refrigerated trailer able to be operational at the John D. Dingell VAMC Waste dock for regulated medical waste storage. The truck's storage area shall be lockable, and the keys shall be left with the COR. The physical quality of this trailer must be high. No damaged, dirty or otherwise unsightly equipment shall be delivered to the work site. Trailers shall be in good operation upon delivery. 10. The collection containers provided by the Contractor for sharps, antineoplastic, cytotoxic, and trace chemotherapy waste shall be compliant with OSHA Bloodborne Pathogen Standards (39CFR 1910.130) and the John D. Dingell VA Medical Center's medical waste management policy and procedures. The Contractor shall also provide additional collection containers upon request for use in an emergency by the COR. 11. All containers shall have appropriate lid to their waste stream that meet VA, Federal and State regulations. The lids will also provide a visual and audible feature to ensure that staff have properly secured the lid to the container. The lid shall also have tamper resistant and secure features to meet federal regulations. 12. Contractor shall provide a summary report of all waste activities using the VHA waste tracking system and a spreadsheet to be provided by the COR. Access to the system will be granted by the COR in EMS. ANTINEOPLASTIC, PATHOLOGICAL, CYTOTOXIC, AND TRACE CHEMOTHERAPY WASTE 1. The Contractor shall provide yellow leak-proof plastic containers that meet state and federal requirements for routine accumulation and collection of discarded gloves, gowns, sharps tubing, and any other disposable materials contaminated with hazardous drugs, blood, and body fluids. Bulk packaging at our shipping site should be of appropriate size and construction to meet VHA and DOT regulations regarding the shipment of this waste stream Desktop containers with secure lids for sharps and other small item collection (4) Wheeled Mobile step on carts of plastic or other appropriate material construction other than metal or wire. To hold floor containers. Mobile carts shall provide: Handsfree operations Opens and closes secure lid by stepping on the cart device Easily able to change from full containers to empty containers without contaminating the environment or staff 2. The Contractor shall incinerate the antineoplastic, cytotoxic and pathological waste in compliance with state regulations. 3. The Contractor shall provide the COR manifests showing the disposal records of all antineoplastic waste picked up from VA facilities under this contract within three weeks of pickup. SHARPS 1. The contractor shall provide Mobile Sharps Collectors/ Trollies for the use of staff to collect sharps in areas identified by the COR. (16) Mobile Sharps Trollies shall be provided for the collection of sharps. Mobile Sharps Trollies shall not be made of metal or wire. Trollies shall be hands free operations with lockable wheels. Lids shall be able to lock and unlock with a final locking mechanism for packaging and final transporting for destruction. Trollies should resist tipping of collector Trollies must be able to accommodate the existing sharps containers that the medical center uses which are BD. GENERAL 1. The contractor shall assist with emergency operations by automatically scheduling waste pickups prior to known winter or other weather storms which could prevent pickup of waste on schedule or when wind weather events are approaching the area. Reducing chances of the infectious waste trailer from becoming compromised and spreading infectious waste into the environment. An empty refrigerated trailer shall be left to ensure waste will not be held inside the facility. 2. Contractor shall provide quarterly training to government staff as required by the COR. Training shall include proper handling of medical/special waste; interim storage requirements, proper identification and labeling of wastes, manifests etc., Training shall include OSHA certification at the end of training 8 hour refresher courses will also be conducted annually for those who are certified. Additionally, courses will be conducted throughout the year for new employees of the medical center and off site locations. 3. Contractor shall provide quarterly summary reports giving analytics concerning the medical waste stream. This report minimally include the following information: a. State of Michigan License/ registration number b. breakdown of each waste stream including amounts transported for the quarter, trainings conducted for the medical center with total students served.
