- SOURCES SOUGHT NOTICE ONLY This notice does NOT constitute a request for proposal, request for quote, or invitation for bid. No formal solicitation document exists at this time. We are seeking information for market research purposes only. The Government will not pay for any information solicited The VHA Network Contracting Office 6 (NCO 6) is conducting a market survey to find qualified vendors who can provide the following equipment per the description of item(s) below for the Department of Veterans Affairs, Richmond VA Medical Center (VAMC), 1201 Broad Rock Blvd, Richmond, VA 23249: DESCRIPTION OF ITEMS Items: 3D Printer System (Please See Attachment) Manufacturer: Prosthetic Design Inc (PDI) Statement of Work Scope. The Contractor shall provide all labor, personnel, equipment, tools, material, supervision and other items and services necessary to furnish PDI printer for 3D O&P/REHAB. Contractor Responsibilities: to maintain and upkeep the printer s maintenance schedule, and day to day operations. Should also have a functional working lab with tools and equipment to assist in socket fabrication. Specific Tasks. The Vendor will provide the following supplies: PDI-Squirt Shape 3-D printer PC, Monitor OS software PDI Polypropylene Impact, printing material 50lbs 3-D Lock Cup Base-50 pieces On Site, Technical Training (2 days) Lamination tooling Vendor will also set up printer and explain maintenance schedule. Interested firms are reminded that in accordance with FAR 4.1201(a) Prospective contractors shall complete electronic annual representations and certifications at the System for Award Management (SAM) as a part of required registration in conjunction with FAR 4.1102(a) that states, Prospective contractors shall be registered in the (SAM) database prior to award of a contract or agreement. Therefore, due to time constraints, vendors will be required to be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award.
