Description
Functional/Performance/Specification Ver.15DEC2017 General Equipment Name: Transport Chair (2237 636-19-3-3998-0599) Description: The VA Central Iowa Health Care System (VACIHCS) has a need for more ergonomic, clinical transport chairs. We are currently requesting 10 chairs, which plan to be kept manly in the ED and OR areas. These new transport chairs are much more durable than collapsible wheelchairs, are much less likely to leave the facility due to theft, are more easily identifiable (can be color coded to each specific area), nest/stack, and are easily cleaned (can be wiped down with Saniwipe or power washed). They offer more patient comfort with easier ingress and egress capacity. They are more ergonomically friendly for the caregiver with push button actuator points for all positioning features. These chairs require a caregiver to push the veteran and don t allow the veteran to self-transport. There are also no removable parts- making this chair appropriate for all care environments and contributes to more time in-service because of not having to locate missing parts-pieces. These chairs also accommodate a 500- pound weight limit with easier steer and brake capacity with wider seat width than standard transport chairs. Salient Characteristics (at least 5), The item must: be/ have/ do these things The chairs must be non-foldable to prevent theft. The chairs must have anti-tip wheels or similar features for patient safety. The chairs must be able to be stacked and nested together to minimize storage requirements, a similar concept to shopping carts. The chair must be able to withstand power washing and not have seams for infection control purposes. The chair must be able to accommodate a load of at least 500 pounds. The moveable parts, such as arm rests and foot rests must be able to swing out of the way to accommodate transferring the patient safely, but not be able to be removed from the chair, to prevent loss of the part. The braking mechanism should not require continual compression by the caregiver/transporter to operate; instead it should be engaged or disengaged with one touch either by the hand or foot. Ingress and Egress should be from the front of the chair, adding to patient safety as this is intuitive to patients/families Evaluation Criteria Technically acceptable (meets or exceeds salient characteristics) Stryker Prime TC - Brand name or equal Small business consideration Service Disabled Veteran Owned Small Business; SDVOSB, and Veteran Owned Small Business; VOSB Cost Required Individual Line Items & Quantity of Items Item # Description/Part Number* Qty 1 Prime TC Swing-Away Model/1460000000 10 2 Prime TC Swing Away Com Comps/1460016000 10 3 Padded seat option/1460013050 10 4 Seat Back Plaque Lable Yellow/140013029 10 5 Foley Bag Book Option w/ leg rest/1460001200 10 6 IV pole option/1460035000 10 7 IV Pole Topper, Yellow/1460035029 10 8 O2 Holder/1460145000 10 9 Leg rest option/1460166030 10 10 Prime TC Domestic Ops Manual 10 11 Packaging, unboxed 10 12 3 yr parts, 1 yr labor and travel - (1 each) Maintenance Manuals (CD, PDF or Word) (1 each) User Manuals (1 each) Cleaning Instructions for all equipment, supplies and accessories 3 yr parts, 1 yr labor and travel Optional Line Items no trade in for this requirement.
