- The Department of Veterans Affairs intends to negotiate a sole source contract to purchase one (1) Zeiss Humphrey Visual Field Analyzer 3, Model 860 (HFA3 Model 860) item number 266002-1170-747-PRO with Liquid Trial Lens, RelEye, Kinetic, GPA, HeadTrk, Gaze Trk, Printer and Table. The Chillicothe VA Medical Center will continue standard of care in their Eye Clinic and also at their associated CBOCs by using the Zeiss software currently in use at the Chillicothe VA. The requested HFA3 Model 860 is currently only compatible with our current FORUM Server. The North American Industry Classification System Code (NAICS) is 339115, Ophthalmic Goods Manufacturing. The Government believes that Zeiss and their authorized Small Business (SB) Distributors are the only providers that can satisfy the agency requirements (Zeiss does not sell directly to the VA). Therefore, the Government intends to proceed with a sole source award for the required HFA3 Model 860. This procurement is being conducted in accordance with FAR 6.302 only one responsible source and no other vendor will satisfy agency requirements. To the best of our knowledge, Zeiss and its authorized SB Distributors are the only sources that provides the above indicated services/supplies. This notice is to request competitive quotes only from the authorized SB Distributors. No solicitation document is available; however, any firm that believes it can meet these requirements may give written notification to the Contracting Officer within seven (7) calendar days from the date of publication of this synopsis. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to comply with the above requirements. Information can be sent to the Contracting Officer, VA Ambulatory Care Center, 420 N. James Road, Columbus, Ohio 43219-1834 or faxed to 614.388.7500 or emailed to gina.crank@va.gov. Responses received will be evaluated however a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses, other than those of the authorized SB Distributors are received, the Chillicothe VA Medical Center will proceed with the sole source negotiation with the authorized SB Distributor Vendor that offers the lowest priced technically acceptable (LPTA) Quote. DEPARTMENT OF VETERANS AFFAIRS Justification for Single Source Awards IAW FAR 13.106-1 For Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K) Acquisition Plan Action ID: 36C250-19-AP-4648 Contracting Activity: Department of Veterans Affairs, VISN/NCO 10, Chillicothe VA Medical Center, 17273 State Route 104, Chillicothe, Ohio 45601. Transaction # 538-19-3-087-0043 Brief Description of Supplies/ Services required and the intended use/Estimated Amount: Purchase of the Zeiss HFA3 Model is necessary to continue standard of care in our Chillicothe Eye Clinic and associated CBOCs. The current visual field machines are either broken and beyond repair or are past their EOL date. This is a critical piece of equipment that all eye care professionals use on a daily basis in each of our Clinics. Requesting 1ea for the Chillicothe VAMC and their associated CBOCs: Zeiss HFA3 Model 860w/Liquid Trial Lens, RelEye, Kinetic, GPA, Head TRK, Gaze TRK, Printer and Table, Installation and Training x 1. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The HFA3 MODEL 860 is specifically requested due to the ability to integrate with the specialized equipment currently in the Chillicothe VAMC Eye Clinics. No other Brands of field machines other than Zeiss is compatible with our current FORUM Server and existing historical patient data. A change to a different manufacturer would result in loss of the ability to directly evaluation progression data and potentially decrease the quality of care and patient efficiency. Description of market research conducted and results or statement why it was not conducted: Market Research was conducted by search of VIP, GSA, FPDS and the Internet. As mentioned, Zeiss is the sole proprietor of this requirement. Zeiss does not sell directly to the VA; the purchasing is done from their authorized SB Distributors. This requirement will be purchased from their authorized SB Distributor that offers the lowest price technically acceptable to the VA.
